Two cases diagnosed as chronic renal failure after fundus examination

Abstract

Diffuse macular edema, preretinal hemorrhages and soft exudates at fundus, sclerotic changes at retinal arterioles, venous dilatation, Gunn sign in both eyes of two cases and macular star in the right eye of one case were detected in two cases that applied to our clinic with decreased vision. There were leaking spots that increases in the late phase in the fundus fluorescein angiography(FFA). Findings were consistent with high grade hypertensive retinopathy. Urea and creatinine levels were found to be increased in laboratory investigations that made in consequence of systemic hypertension in one case and fatigue in both cases. Two cases that were referred to internal medicine department were taken into hemodialysis program with the diagnosis of chronic renal failure. After hemodialysis vision increased in both cases and fundus findings were regressed.

Görme azlığı şikayeti ile kliniğimize başvuran iki olguda her iki gözde diffüz makula ödemi, arka kutupta retina önü kanamalar ve yumuşak eksudalar, retina arteriyollerinde sklerotik değişiklikler, venlerde dolgunluk, Gunn arazı ile bir olgunun sağ gözünde makula yıldızı saptandı. Fundus fluoresein anjiyografide (FFA) arka kutupta geç dönemde artan sızıntı odakları mevcuttu. Bulgular ileri derece hipertansif retinopati ile uyumlu idi. Bir olguda sistemik hipertansiyon öyküsü ve her iki olguda halsizlik mevcut olması sebebiyle yapılan laboratuar incelemelerinde üre ve kreatinin değerleri yüksek bulundu. Dahiliye bölümüne yönlendirilen iki olgu kronik böbrek yetmezliği (KBY) tanısı ile hemodiyaliz programına alındı. Hemodiyaliz sonrasında iki olguda görme keskinliği arttı ve göz dibi bulguları geriledi.
