Results of Surgery in Patients with type 1 Duane’s Retraction Syndrome

Abstract

Purpose: To assess the results of surgery techniques used in Duane’s retraction syndrome (DRS) type 1. Material and Method: 194 DRS cases followed in the department of strabismus were retrospectively investigated. We assessed 58 cases operated for esotropic DRS type 1 in terms of pre- and post-operative deviations in primary position, abnormal head posture (AHP), abduction deficiency, globe retraction, and up-down shoot. The cases were divided into 3 groups according to the surgical technique applied. The mean follow-up period was 22.4±4.6 (1-13 years) months. Results: The cases in group 1 had undergone uni- or bilateral medial rectus (MR) recession, and the postoperative 1stmonth improvement rates for distance deviation, near deviation and AHP were 68.6%, 73.9% and 50%, respectively. These rates were 69.3%, 64.7% and 57.1%, respectively in group 2 which consisted of eyes that had undergone vertical rectus muscle transposition (VRT) surgery. In group 3, in which we had performed Y-split (with or without LR/MR recession) surgery, the improvement rates for distance and near deviation were 63.6% and 63.9%, and 50% was determined for AHP. The improvement in globe retraction was 60% in group 1 and 66.6% in group 3. Discussion: Medial rectus muscle recession was more effective for correction of deviation and AHP in cases of DRS type 1. VRT was found to be more effective surgical option in abduction deficiency. Y-split surgery has an important place in eliminating globe retraction and up/down shoot. Amaç: Tip 1 Duane retraksiyon sendromu(DRS) olgularında uygulanan cerrahi tekniklerin sonuçlarını değerlendirmek. Gereç ve Yöntem: şaşılık bölümünde takip edilen 194 DRS’lu olgu retrospektif olarak incelendi. Cerrahi uygulanan ezotropik tip 1 DRS’lu 58 olgu; operasyon öncesi ve sonrası primer pozisyondaki kayma miktarı, anormal baş pozisyonu (ABP), abduksiyon kısıtlılığı, glob retraksiyonu ve aşşagı/yukarı atımları açısından değerlendirildi. Olgular uygulanan cerrahi tipine göre 3 gruba ayrıldı. Olguların ortalama takip süresi 22,4±4,6 ay (1-13 yıl) idi. Sonuçlar: Bir veya her iki MR’lara geriletme uygulanan grup1’de, postoperatif 1. ayda uzak kaymanın %68,6, yak›n kayman› n %73,9 ve ABP’nun %50 oranlar›nda düzeldi¤i, VRT cerrahisi uygulan grup 2 de; bu oranlar sırasıyla %69,3, %64,7 ve %57,1 olarak saptanmıştır. Y split (LR/MR geriletmesi ile beraber olan/olmayan) cerrahisi uygulanan grup 3’de ise uzak kayman› n %63,6, yak›n kaymanın %63,9 ve ABP’nun %50 oranında düzeldiği saptandı. Glob retraksiyonundaki düzelme grup1’de %60, grup3’te %66,6 olarak bulunmuştur. Tartışma: Tip 1 DRS’nda medial rektus kaslarının geriletilmesi, primer pozisyondaki kayma ve ABP’unun giderilmesinde etkilidir. VRT ise özellikle abduksiyon kısıtlılıgın giderilmesinde etkilidir. Y split cerrahisi, glob retraksiyonunun ve yukar›/aşşagı atımın düzeltilmesinde önemli etkiye sahiptir. (Turk J Ophthalmol 2011; 41: 156-63)
