Surgical Results in Cases of Sensory Strabismus

Abstract
Purpose: To determine horizontal deviation type and to evaluate the correlation between deviation type/etiology and surgical results for sensory strabismus. Patients and Methods: The reports of 29 patients operated for sensory strabismus (12 female, 17 male) whose mean age was 22.17±11.52 (range: 4-57) years were evaluated retrospectively. Sixteen cases (55.2%) had exotropia (XT) and 13 cases (44.8%) had esotropia (ET). Etiologies, ages during surgeries, and preoperative/postoperative deviation amounts were noted for the total of the patients as well as for ET and XT groups separately. The results for ET and XT groups were compared statistically using t test. The mean follow-up time was 4.27±3.5 years (range: 4 months-12 years) and deviation in ±10 prism diopters (PD) in the last visit was considered as success. Results: Etiologies in all cases examined were as follows: anisometropia in 13 (44.8%), trauma in 10 (34.5%), congen- ital cataracts in 2, and congenital glaucoma, keratoconus, choroidal coloboma, and hypoplastic optic disc in one case each. The visual acuity of the squinting eyes ranged from no light perception to 0.8 logMAR. The mean preoperative deviation was 46.24±19.29 PD, and the mean postoperative deviation decreased to 9.55±11.86 PD in the last visitWhen the ET and XT groups were compared, the congenital causes were more common in the ET group (30.75%) com- pared to the XT group (6.25%), otherwise, there was no statistically significant difference in terms of mean age, preop- erative and postoperative deviation amounts and follow-up time between the two groups (p>0.05). In contrast, while the surgical success rate was found to be 75.9 % for all cases and 87.5% for the XT group, it was 61.5% for the ET groupDiscussion: Despite the deep amblyopia in sensory strabismus, satisfactory surgical results are achieved; nevertheless, the success may be more limited in sensory esotropia particularly due to congenital causes. Giriş: Sensoryel şaşılıklarda görülen horizontal kayma tiplerini belirlemek, kayma tipi ile kaymaya yol açan etyoloji ve cerrahi sonuçlar arasındaki ilişkiyi değerlendirmek. Yöntem ve Gereç: Sensoryel şaşılık nedeniyle opere edilmiş, yaşları ortalama 22,17±11,52 yıl (4-57) olan 12si kadın, 17si erkek 29 hastanın kayıtları retrospektif olarak incelendi. Olguların 16sında (%55,2) ekzotropya (XT), 13ünde %44,8) ezotropya (ET) mevcuttu. Hem bütün olgular için hem de ET ve XT grupları için ayrı ayrı etyolojiler, cerrahinin yapıldığı yaş, cerrahi öncesi ve son kontroldeki kayma derecesi ortalamaları kaydedildi. ET ve XT alt gruplarının sonuçlarıtesti kullanılarak istatistiksel olarak birbiriyle karşılaştırıldı. Ortalama 4,27±3,5 yıl (4 ay-12 yıl) takip edilen hastaların kayma derecesi son kontrolde ±10 PD aralığında ise başarılı kabul edildi. Sonuçlar: Tüm olgularda etyolojiler incelendiğinde, 13ünde anizometropi (%44,8), 10unda travma (%34,5), 2sinde konjen- tal katarakt, birer olguda da konjenital glokom, keratokonus, koroid kolobomu ve hipoplastik optik disk saptandı. Hastaların kayma olan gözlerinde görme derecesi ışık hissi (-) ile 0,8 logMAR arasında değişmekteydi. Cerrahi öncesi ortalama kayma miktarı 46,24±19,29 prizm diyoptri (PD) iken cerrahi sonrası son kontrolde 9,55±11,86 PDye düşmüştü. ET ve XT grupları karşılaştırıldığında, ET grubunda etyolojide konjenital nedenlerin (%30,75) XT grubuna (%6,25) göre daha yüksek bulunması dışında; yaş, cerrahi öncesi ve sonrası kayma ortalamaları ve takip süresi açısından istatistiksel olarak anlamlı fark bulunmadı p>0,05). Buna karşılık, cerrahi başarı oranı bütün hastalar için %75,9, XT grubu için % 87,5 iken ET grubunda %61,5 bulundu. Tartışma: Sensoryel kaymalarda derin ambliyopiye rağmen oldukça tatminkar cerrahi sonuçlar elde edilmekle beraber özellikle konjenital nedenlere bağlı gelişen sensoryel ezotropyalarda başarı daha sınırlı olabilmektedir.
