Toxic anterior segment syndrome (TASS)

Abstract
Toxic anterior segment syndrome (TASS) is a sterile intraocular inflammation caused by noninfectious substances, resulting in extensive toxic damage to the intraocular tissues. Possible etiologic factors of TASS include surgical trauma, bacterial endotoxin, intraocular solutions with inappropriate pH and osmolality, preservatives, denatured ophthalmic viscosurgical devices (OVD), inadequate sterilization, cleaning and rinsing of surgical devices, intraocular lenses, polishing and sterilizing compounds which are related to intraocular lenses. The characteristic signs and symptoms such as blurred vision, corneal edema, hypopyon and nonreactive pupil usually occur 24 hours after the cataract surgery. The differential diagnosis of TASS from infectious endophthalmitis is important. The main treatment for TASS formation is prevention. TASS is a cataract surgery complication that is more commonly seen nowadays. In this article, the possible underlying causes as well as treatment and prevention methods of TASS are summarized. Toksik anterior segment sendromu (TASS) cerrahi sonrasıön segmentte infeksiyöz olmayan ajanlarla gelişen ve göz içi dokularda yaygın hasarla sonuçlanan göz içi inflamasyondur. Etyolojik faktörler arasında cerrahi travma, bakteriyel endotoksinler, göz içi kullanılan solüsyonların uygun olmayan pH ve osmolalitesi, koruyucular, denatüre oftalmik viskocerrahi gereçler (OVD), cerrahi malzemelerin yetersiz durulanması, temizlenmesi, sterilizasyonu, göz içi mercekler, göz içi merceklerde kullanılan temizleme ajanları ve cilalama bileşikleri sayılabilir. Bulanık görme, korneal ödem, hipopyon ve reaktif olmayan pupilla ile karakterize bulgu ve semptomlar genellikle ilk 24 saatte ortaya çıkar. İnfeksiyöz endoftalmi ile ayırıcı tanısıönemlidir. TASS oluşumunun önlenmesi en önemli tedavi seçeneğidir. TASS günümüzde giderek sık görülen katarakt komplikasyonlarından biridir. Bu nedenle olası TASS sebepleri, tedavi seçenekleri ve korunma yöntemleri bu yazıda özetlenmiştir. (Turk J Ophthalmol 2011; 41: 407-13)
