Combined surgical treatment of ocular complications developing from facial nerve palsy: Case Report

Abstract

A 71-year-old man who had developed right facial nerve palsy after acoustic neuroma resection was referred to our eye clinic with ocular complications. Due to the brow ptosis, he could not open his right eye properly. There were ocular surface problems as a result of incomplete closure of the upper eyelid over the cornea. He complained of epiphora resulting from paralytic ectropium. Surgical procedures performed at the same session were right brow suspension, 0.8 g gold weight implantation into the right upper eyelid, lateral canthoplasty, and horizontal lid shortening of the right lower eyelid. After the surgery, improvements of his complaints were satisfying. As a conclusion, in the treatment of more than one ocular complication developing from facial nerve palsy, combined surgical procedures may be performed in terms of both functional and aesthetics improvements. Özet

Akustik nörinom rezeksiyonu sonrası sağ fasial sinir felci gelişen 71 yaşındaki erkek hasta, oküler komplikasyonlar ile kliniğimize başvurdu. Kaş ptozisine bağlı sağ gözünü rahat açamıyordu. Gözünü tam kapatamadığı için oküler yüzey problemleri vardı. Alt kapakta gelişen paralitik ektropium nedeniyle göz sulanmasından şikayetçiydi. Tedavisinde aynı cerrahi seansta, sağ kaş kaldırma cerrahisi, sağ üst göz kapağına 0,8 gr altın ağırlık yerleştirilmesi, lateral kantoplasti ve yatay sağ alt kapak kısaltması işlemleri uygulandı. Cerrahi sonrası yakınmalarındaki iyileşme tatminkardı. Sonuç olarak, fasial sinir felci sonrası gelişen birden fazla oküler komplikasyonun tedavisinde, fonksiyonel ve estetiksel iyileşmeyi sağlayabilecek kombine cerrahi işlemler uygulanabilir.
