A case of acute bilateral syphilitic uveitis

Abstract

We aim to present the case of a patient with syphilitic uveitis whom dermatological and neurological examinations were normal. He presented with gradually worsening visual acuity in both eyes and both venereal disease research laboratory and microhemagglutination assay for Treponema pallidum serological tests were confirmatory for syphilis. In posterior segment examination, an uncommon manifestation of syphilitic uveitis and posterior placoid chorioretinitis was present in both eyes, however the left eye posterior segment inflammation was much worse in comparison to the fellow eye. Ocular manifestations of syphilis have a myriad of presentations, and the re-emergence in the last decades obligate us to keep in it mind in unexplained ocular inflammatory diseases even if patient history is not compatible. Özet

Bu olguyla başka bir sistemik rahatsızlığı olmayan dermatolojik veya nörolojik yönden de herhangi bir pozitif bulgusu tespit edilemeyen ilk ve tek olarak oküler bulgu veren bir sifiliz olgusunu sunmayı amaçladık. Her iki gözde bulanık görme şikayetleriyle gelen ve sağda posterior solda ise panüveiti bulunan hastada sifilizin varlığını venereal disease research laboratory ve treponema hemaglutinasyon testi gibi serolojik testlerle tespit ettik. Hastanın sol gözünde diğer gözüne göre çok daha ağır bir inflamasyon tablosu mevcut olsa da her iki gözünü de tutan sifilitik üveitin nadir görülen bir varyantı olan posterior plakoid koryoretinit tablosu mevcut idi. Sifilizin oftalmolojik yönden hem ön hem de arka segmentte çok farklı formlarda klinik antitelere yol açabilmesi ve son yıllarda yaşam koşullarına bağlı olarak insidansında görülen artış nedeniyle sebebini açıklayamadığımız oküler inflamatuvar olgularda hastanın hikayesi bununla uyumlu olmasa da sifiliz ihtimalini mutlaka aklımızda tutmalıyız.
