CRH stimulation test and petrosal sinus sampling for the diagnosis of Cushing's disease: Case report

Abstract
The clinical, biochemical, and radiographic features of ACTH-dependent hypercortisolism (Cushing's disease) are often indistinguishable from those of ectopic ACTH-dependent Cushing's syndrome. Bilateral inferior petrosal sinus ACTH sampling with CRH stimulation is a useful test for differential diagnosis and preoperative location of pituitary microadenomas. This report presents a patient with Cushing's disease diagnosed by inferior petrosal sinus sampling and CRH stimulation, and treated surgically.
Özet
ACTH'ya bağlı hiperkortizolizm (Cushing hastalığı) ile ektopik ACTH salınımına bağlı Cushing sendromunun klinik, biyokimyasal ve radyolojik bulguları genellikle birbirinden ayırt edilememektedir. CRH stimülasyon testi ile birlikte bilateral inferior petrozal sinüs örneklemesi, ayırıcı tanıda ve hipofiz mikroadenomlarının cerrahi öncesi lokalizasyonunda yararlı bir testtir. Bu olgu sunumunda, CRH stimulasyon testi beraberinde inferior petrozal sinüs örneklemesi yapılarak Cushing hastalığı tanısı koyulup, cerrahi olarak tedavi edilen bir hastadan ve konuyla ilgili literatürden bahsedildi.
