The Place of Dynamic and Semidynamic Systems in Degenerative Lumbar Spine

Abstract

Degenerative disc disease is one of the most important causes of back pain. Pathological movements in the deformed moving segment cause pain. For this reason, the destruction of the motion of the segment also eliminates the pain. Degenerative unstable primary treatment options are fusion, dynamic stabilization and lumbar arthroplasty. Posterior dynamic stabilization in surgical treatment of degenerative instability is the preferred surgical method and the mortality and morbidity rates are low. Dynesys is the most widely used dynamic system in the world today. When applying dynamic systems, dynamic or semi-dynamic stabilization systems that best mimic the appropriate movement of the spine should be preferred. More research and more advanced systems are needed for this purpose.

Dejeneratif disk hastalığı bel ağrısının en önemli sebeplerinden biri olarak gösterilmektedir. Dejenere olan hareketli segmentte meydana gelen patolojik hareketler ağrıya neden olmaktadır. Bu nedenle, segmentin hareketinin yok edilmesi ağrıyı da yok etmektedir. Dejeneratif instabil omurgada başlıca tedavi seçeneklerini; füzyon, dinamik stabilizasyon ve lomber artroplasti oluşturmaktadır. Dejeneratif instabilitenin cerrahi tedavisinde posterior dinamik stabilizasyon, ilk tercih edilmesi gereken cerrahi yöntem olup, mortalite ve morbidite oranı düşüktür. Dynesys, günümüzde dünya genelinde en çok kullanılan dinamik sistemdir. Dinamik sistemler uygulanırken omurganın yaşına uygun hareketini en iyi taklit eden dinamik veya yarı dinamik stabilizasyon sistemleri tercih edilmelidir. Bu amaç için daha çok çalışma ve daha çok gelişmiş sisteme ihtiyaç vardır.
