Abstract

The well-being of patients after spinal surgery depends on many factors that are present before, during and after the operation. These factors may be related to the surgical techniques and planning. In addition to age, body-mass index, spinal structure and co-morbidities of the patient, surgical complications such as problems with fusion application, insufficient neural decompression, dural damage, and screw malposition can also cause failed back surgery syndrome. In the postoperative stage, breakage of the screw, malposition of the fusion material, pseudoarthrosis, or surgical site problems such as infections or hematoma may trigger the condition. The management of patients who have not been treated at the desired level after lumbar spinal surgery has become a complicated subject that continues to maintain its importance.

Özet

Omurga cerrahisi sonrasında hastanın iyilik hali operasyon esnasında, öncesinde ve sonrasında mevcut olan birçok etkene bağlıdır. Bunları açmak gerekirse; hastaya bağlı sebepler olabileceği gibi cerrahi teknik ve planlamalara da bağlı olabilmektedir. Hastanın yaşı, beden-kitle indeksi, omurga yapısı, eşlik eden ko-morbiditelerinin yanı sıra; füzyon uygulamasındaki sıkıntılar, yetersiz nöral dekompresyon, dura hasarı, vida malpozisyonu gibi cerrahi komplikasyonlar da bu sendromun oluşmasına zemin hazırlamaktadır. Postoperatif dönemde vida sıyrılması veya kırılması, füzyon materyali malpozisyonu, psödoartroz veya yara yerine ait enfeksiyon, hematom gibi problemler de durumu tetikleyebilmektedir. Lomber omurga ameliyatı sonrası istenilen düzeyde sağaltım sağlanamayan hastaların yönetimi her dönem önemini koruyan komplike bir konu olmuştur.
