The Classification of Central Nervous System TumorsFrom Past to Date

Abstract

The World Health Organization has published four editions of the Classification of Central Nervous System (CNS) Tumors in the years 1979, 1993, 2000, 2007, and the updated 4th edition in 2016. However, the knowledge on brain tumors is being renewed almost every day. The International Society of Neuropathology (ISN) has decided to publish some guidelines for the current neuropathology practice until a new blue book comes out and has announced cIMPACT-NOW (the Consortium to Inform Molecular and Practical Approaches to CNS Tumor Taxonomy). The group has released 4 updates in the last 2 years. As the cIMPACT-NOW updates were published, the number of molecular tests required for CNS tumors has increased rapidly. It is not possible to disregard these suggestions as in case of a tumor with low-grade morphology, recurrence and mortality could be seen in a very short time and the pathological diagnosis was far from explaining the condition. Following the updates closely, putting them into practice, and taking the recommendations into consideration have made a great contribution to clinicopathological compliance. The first round has been completed, and working groups are now preparing the second round. A blue book for the new classification is expected to be published by the end of 2020

Dünya Sağlık Örgütü Santral Sinir Sistemi Tümörlerinin Sınıflaması üzerine 1979, 1993, 2000 ve 2007 yılları dahil olmak üzere 4 ayrıedisyon, 2016 yılında ise güncellenmiş 4. edisyonu yayınlamıştı. Ancak bilgiler her geçen gün yenileniyor, bazı doğru kabul edilenleryeni bilgilerle geçerliliğini yitiriyordu. Yeni bir mavi kitap basılana dek Uluslararası Nöropatoloji Topluluğu (ISN), bazı yenilikleri güncelnöropatoloji pratiğinde kullanmaya rehberlik edecek makaleler halinde basıp yayınlamaya karar verdi. ISN tarafından duyurulancIMPACT-NOW (The Consortium to Inform Molecular and Practical Approaches to CNS Tumor Taxonomy) bir sonraki mavi kitapbasılana dek nöropatolog, nöroonkolog ve nöroşirürjiyenlere yol gösterecekti. Grubun 2018’de 3 ve 2019’da 1 güncellemesiyayınlandı. cIMPACT-NOW güncellemeleri yayınlandıkça santral sinir sistemi tümörlerinde uygulanması gereken moleküler testsayısı hızla artıyordu. Bu önerileri dikkate almamak ise mümkün değildi. Zira morfolojiye dayalı olarak düşük dereceli tümör tanısıverilen, bu tanıya dayalı tedavi ve takip planı oluşturulan bir olguda çok kısa sürede nüks ve mortalite görülebilmekte ve patolojik tanıbu durumu açıklamaktan uzak kalmaktaydı. Dolayısıyla güncellemeleri yakından takip etmek, önerileri dikkate alarak uygulamayakoymak klinikopatolojik uyuma büyük katkı sağlamaktaydı. Birinci turu tamamlanan seride şimdi çalışma grupları 2. tur üzerinehazırlıklarını sürdürmekteler. Tahminen 2020 sonlarında ise yeni sınıflama mavi kitabının basılması bekleniyor.
