The Anatomy of the Petrous Bone and Anterior Petrosectomy

Abstract
The temporal bone has a very important role in skull base surgery. Knowing the anatomy of the petrous section is especiallyimportant in the surgery of the petrous apex and superior clival region lesions. The petrous bone has 3 parts: the apex, base and the 3 surfaces. Anterior petrosectomy (extended middle fossa approach) is evaluated in 5 sections: 1. Detection of the middlemeningeal artery, and coagulation and division, 2. Detection of the foramen ovale and V3 (mandibular nerve) passing through it, 3.Detection of the GSPN (greater superficial petrosal nerve) and dissecting it from the dura, 4. Detection of the meatal plane in the petrous ridge (projection of IAC) and trigeminal impression, 5. Anterior petrosectomy. This technique is safe for the petrous apex, clivus and posterior fossa lesions. The limitations should be considered.
Özet
Temporal kemiğin, kafa tabanı cerrahisinde çok önemli bir görevi vardır. Özellikle petröz bölümünün anatomisini iyi bilmek petröz apeks ve süperior klival bölge lezyonlarının cerrahisinde çok önemli bir yer tutmaktadır. Petröz kemik, taban, apeks ve 3 yüzeyden oluşur. Anterior petrözektomi (extended orta fossa cerrahisi) ;1. Orta meningeal arterin tespiti ve koagule edilip, ayrılması 2. Foramen ovalenin ve buradan gecen V3’ (mandibular sinir) un tespiti 3. GSPN (greater superficial petrosal nerve)’in tespiti ve temporal duradan diseke edilmesi 4. Petröz sırttaki meatal planın ( IAC projeksiyonu) ve trigeminal oluğun tespiti ve 5. Anterior petrözektomi olarak 5 bölümde incelenir. Bu teknik petröz apeks, klivus ve posterior fossa lezyonlarında güvenli bir tekniktir. Limitasyonları göz önünde bulundurmalıdır.
