The Anatomy of the Insular Region and the Surgical Approach Routes

Abstract
The insula has been the subject of research by neurosurgeons for many years in terms of its anatomy, functions and pathologies.Surgical interventions of the insular region have been risky particularly because of the lack of direct contact with the cerebralsurface, being covered by the frontal, parietal and temporal operculums, neighboring deep structures, the relationship with theimportant vascular structures and the complexity of its functions. For many years, the surgery of insular region pathologies hasbeen avoided and this has led to the development and discussion of various approaches. Recently, the approaches for regionalpathologies have shifted from conservative to aggressive with the help of associated techniques such as intraoperative magneticresonance, neuronavigation, functional brain mapping and neurophysiological studies via direct electrical stimulation. The aim ofthis article is to discuss the anatomy of the insular region and the surgical approaches.
Özet
İnsula uzun yıllardan beri anatomisi, fonksiyonları ve patolojileri açısından beyin cerrahları tarafından araştırmaların konusu edilmiştir. İnsula beyin yüzeyi ile direkt ilişkisinin bulunmayan, silvian fissür zemininde fronral, parietal ve temporal operkulumlar tarafından örtülmüştür. Derin yapılarla komşulukları, önemli vasküler yapılar ile olan ilişkisi ve fonksiyonlarının karmaşık yapısı nedeniyle bölge patolojilerine yönelik cerrahi girişimleri riskli hale getirmektedir. Uzun yıllar insular bölge içerisinde yer alan patolojilerin cerrahisinden çekinilmiş ve bu bölgeye farklı yaklaşım yollarının geliştirilip tartışılmasına neden olmuştur. Son dönemlerde intraoperatif manyetik rezonans, nöronavigasyon, direkt elektrik stimulasyon ile fonksiyonel beyin haritalama ve nörofizyolojik incelemeler gibi çeşitli yardımcı teknikler ile bu bölge patolojilerine yönelik yaklaşımlarda konservatif yaklaşımlardan daha agresif yaklaşımlara doğru yönelim gerçekleşmiştir. Makalede insular bölgenin anatomisinin gösterilmesi ve cerrahi yaklaşım yollarının tartışılması hedeflenmiştir.
