Hyperperfusion syndrome after carotid endarterectomy: 5 cases

Abstract
Hyperperfusion Syndrome After Carotid Endarterectomy: 5 Cases Scientific background: Hyperperfusion Syndrome (HS) is a rare but devastating complication which develops after carotid artery surgery or carotid artery stent placement. It is due to the breakdown of autoregu-lation mechanisms which have been adapted to chronically decreased blood flow. Objectives, materials and methods: Five patients between 1987-2004 were diagnosed as HS after carotid endarterectomy (CEA). In this period, 320 CEA were done by the same surgical team. Risk and etio-logical factors, clinical signs, imaging studies and prognostic factors were evaluated. Results: Age of four male and five female patients was between 58-77 years. Four patients had two or more stroke risk factors, the most prominent of which was hypertension. Symptomatic internal carotid artery (ICA) stenosis was present in all patients. Four of them were evaluated because of transient ischemic attacks and another one because of stroke. Three of them had severe occlusive disease of the contralat-eral ICA. HS developed within 1-6 days postoperatively. The most frequent signs were headache, focal seizures and focal neurological deficits. Blood pressure (BP) was high in four of the patients in the postoperative period. ICAs were patent in all after surgery. Neuroimaging findings revealed intracerebral hemorrhage (ICH) in 3 patients. One patient's ICH was associated with the use of anticoagulants. Two of them died. Recovery was within months in the other patient and within days in two non-hemorrhagic patients. Conclusions: Detection of high risk patients and early diagnosis are important in the prevention of this devastating complication. Chronic hemispheric hypoperfusion, stroke, uncontrolled hypertension and anti-coagulation increase the risk. Death and severe sequale can be prevented with the recognition of early clinical signs such as headache, close follow-up of BP and antihypertensive therapy.

Bilimsel zemin: Hiperperfüzyon Sendromu (HS), karotis arter cerrahisi veya stentlemesinden sonra gelişen nadir, fakat ölümcül olabilen bir komplikasyondur ve kronik olarak azalmış kan akımına uyum göstermiş otoregülasyon mekanizmalarının yıkılması sonucu gelişir.Amaç, gereç ve yöntemler: 1987-2004 yılları arasında kliniğimizde 5 hastaya karotis endarterektomisi (KEA) sonrası HS tanısı konulmuştur. Aynı dönem içinde bu olguları öpere eden ekip 320 KEA gerçekleştirmiştir. Bu hastalar risk ve etiyolojik faktörler, klinik bulgular, görüntüleme özellikleri ve prognostik faktörler açısından incelenmiştir. Sonuçlar: 4'ü erkek 5 hastanın yaşları 58-77 yıl arasındaydı. 4 hastada hipertansiyon başta olmak üzere 2 veya daha fazla inme risk faktörü vardı. Hastaların tümünde semptomatik internal karotis arter (İKA) stenozu saptandı. Hastaların 4'ü geçici iskerhik atak, bir diğeri inme nedeniyle değerlendirildi. 3 hastanın karşı İKA'da ileri darlığı vardı. HS, cerrahiyi takiben 1-6. günler arasında gelişti. En sık görülen bulgular başağrısı, fokal nöbetler ve fokal nörolojik defisitti. Hastaların 4'ünde cerrahi sonrası kan basıncı (KB) değerleri yüksekti. Tüm hastaların cerrahi sonrası incelemelerinde girişim yapılan İKA'lar açık bulundu. Kranyal görüntülemede 3 hastada intraserebral kanama (İSK) saptandı. Bir hastada İSK antikoagülan tedavisi altında gelişmişti. Bu hastaların 2'si öldü. Diğer hasta aylar içinde, non-hemorajik seyreden 2 hasta ise günler içinde hızla düzeldi. Yorumlar: Hayatı tehdit eden bu komplikasyondan korunmada, yüksek riskli hastaların belirlenmesi ve erken tanı önem taşır. Kronik hemisferik hipoperfüzyon, geçirilmiş inme, kontrolsüz KB ve antikoagülasyon, riski arttırmaktadır. Erken dönemde başağrısı gibi klinik belirtilerin tanınması, yakın KB takibi ve antihipertansif tedavi ile ağır sekeller ve ölüm önlenebilir.
