The role of proinflammatory cytokines in acute ischemic stroke

Abstract

The Role of Proinflammatory Cytokines in Acute Ischemic Stroke Scientific background: Limited number of studies have been per­formed in order to demostrate the relationship of proinflammatory cytokines and their roles in etiopathogenesis and prognosis of the acute ischemic stroke. Objective: In this study, the relationship between'serum interleukin-6 (IL-6) and tumor necrosis faktor-a (TNF-a) levels (which are proinflam­matory cytokines induced by acute ischemic stroke) on acute neurolo­gic dysfunction and their roles in ischemic subtypes are evaluated. Material and method: 49 patients (27 male, 22 female) with a mean age of 69.7±10A were included. They were diagnosed as acute ischemic stroke by history, neurologic examination and computerized brain tomography (CT). Control grup consisted from 30 individuals (17 male, 13 female) with a mean age of 67.3±7.9. Blood samples for IL-6 and TNF-a levels were taken within the first 24 hours of the acute ischemic stroke. The relationship between serum IL-6 and TNF-a levels snd risk factors in the development of stroke, CRP levels in peripheral blood and prognosis in the early stage was evaluated. Results: Serum IL-6 and TNF-a levels didn't differ significantly in patients when compared to controls. According to NIHSS patients were scored between 0-36. Levels of serum IL-6 and CRP in patients with high score were higher when compared to patients with low score. No sta­tistically difference in serum levels of IL-6 and TNF-a in between aterotrombotics and cardioembolic groups were present. Conclusion: According to our results we can say that there is a correla­tion in between the levels of inflammatory cytokines and and neurolo­gic dysfunction in the early stages of the acute ischemic stroke. Patients having high cytokine levels had high stroke scale scores. Long term fol­low up studies with more patients participation are needed to confirm and settle this issue.

Bilimsel zemin: iskemik inmenin akut döneminde gelişen beyin hasarında, inflamatuvar sitokinlerin etiopatogenezdeki yeri ve hastalığın fonksiyonel prognozu ile ilişkisini gösteren sınırlı sayıda çalışma vardır. Amaç: Bu çalışmada akut iskemik inmenin indüklediği proinflamatuvar sitokinlerden olan serum lnterlökin-6 (IL-6) ve Tumor Necrosis Factor-a (TNF-a) düzeylerinin iskemik jnme alt gruplarındaki yeri ve akut dönem­deki nörolojik disfonksiyon ile ilişkisi araştırıldı. Gereç ve yöntem: Çalışmaya öyküsü, nörolojik muayenesi ve bilgisayarlı beyin tomografisi (BBT) ile akut iskemik inme tanısı olan ve yaş ortalaması 69.7±10.1 yıl olan, 49 hasta (27 erkek, 22 kadın) dahil edildi. Kontrol grubunu, yaş ortalaması 67.3 ± 7.9 yıl olan (17 erkek, 13 kadın) oluşturdu. IL-6 ve TNF-a düzeyleri için, kan örnekleri inmenin ilk 24 saati içinde alındı. Serum IL-6 ve TNF-a düzeylerinin inme oluşum mekanizmasındaki yeri, CRP düzeyleri ve NIHSS ile elde edilen inme skorları ile ilişkisi araştırıldı. Bulgular: Serum IL-6 ve TNF-a düzeyleri iskemik inmeli hastalarda kontrol grubundan farklılık göstermemekteydi. NIHSS'e göre hastalar 0-36 arası skorlandılar. IL-6 ve CRP düzeyleri ile yüksek inme skorları arasında anlamlı bir birliktelik dikkat çekiciydi. Aterotrombotik ve kardiyoembolik inme eti-yolojisine sahip olan hastalarda IL-6 ve TNF-a düzeyleri açısından anlamlı farklılık saptanmadı. Sonuç: Sonuç olarak inflamatuvar sitokinlerden olan serum IL-6 düzeyleri erken dönem nörolojik disfonksiyon ile ilişkili bulundu. Erken dönemde ağır nörolojik defisiti olan hastaların IL-6 ve CRP düzeyleri anlamlı olarak daha yüksekti. Konunun tam olarak netlik kazanabilmesi için daha fazla vakanın katıldığı uzun vadeli izlem çalışmalarına ihtiyaç vardır.
