Clinic, etiology and prognosis in cerebral venous thrombosis

Abstract

Clinic, Etiology and Prognosis in Cerebral Venous Thrombosis Scientific background: Cerebral venous thrombosis is seen rarely according to arterial stroke and effects every age group. Onset and process is variable and usually a good prognosis occurs. Early development of coma and intracranial hemorrhage are bad prognostic factors. In our study, it is aimed to investigate the neurologic deficits, etiological factors, localization and the prognosis of the patients who are hospitalized with the diagnosis of cerebral venous thrombosis in our in-patient clinic. Method: Patients were investigated with routine hematolögic, biochemical and vasculitic tests and cranial MRI and MR venography. Results: Of fifteen patients, W were women and 5 were men. Mean age was 47.2 and age interval was 7 7-80. 5 patients in acute, 6 patients in subacute and 4 patients in chronic stage was admitted to our clinic. Mostly seen complaint was headache (11 patients) and 7 patients had paresis, 6 had epileptic seizures, 6 had visual deficits and 1 had unsteadiness. 9 patients had superior sagittal, 2 had superior sagittal+trasverse, 2 had superior sagittal+rectus+transverse, 1 had rectus+transverse and 7 had transverse sinus thrombosis. 2 patients had subarachnoid hemorrhage, 2 had lobar hematoma, 1 had subdural hematoma and 1 had hemorrhagic infarct. Etiologically 3 cases were in postpartum period and 7 case was pregnant. 1 patient had protein C deficiency, 1 had activated protein C resistancy, 1 had antithrombin III deficiency, 1 had anticardiolipin antibody positivity and 7 had thrombocytosis. There was no etiological resason in 4 of the cases and 7 patient could not be investigated because of the death of the patient in the early period. 9 patients recovered completely, 3 patients recovered partially and 2 patients had permanent visual loss. Conclusion: Cerebral venous thrombosis needs highly suspection in order to define the clinical signs. As the early treatment prevents exitus and severe disability, early diagnosis is very important.

Bilimsel zemin: Serebral venöz trombozu (SVT) arteryel inmeye oranla daha seyrek görülmekle birlikte her yaş grubunu etkileyebilmektedir. Başlangıç şekli ve seyri değişken olup genellikle iyi prognoza sahiptir. Erken gelişen koma ve intrakranial kanama ise kötü prognostik faktörlerdendir. Çalışmamızda kliniğimizde SVT tanısı ile izlenen olguların nörolojik bulguları, etyolojik faktörleri ve prognozları incelenmiştir. Yöntem: Olgulara rutin hematolojik ve biyokimyasal tetkikler, vaskülit testleri ile kranial MRI ve MR venografi tetkikleri yapılmıştır. Sonuçlar: 15 olgunun 10'u kadın, 5'i erkek idi. Yaş ortalaması 47.2, yaş aralığı 17-80 olup 5'i akut, 6'sı subakut, 4'ü kronik dönemde başvurdu. En sık yakınma başağrısı olup (n=11), 7'sinde kuvvet kaybı, 6'sında epileptik nöbet, 6'sında görme bozukluğu, Tinde dengesizlik mevcuttu. 9 olguda superior sagital, 2 olguda superior sagital+transvers, 2 olguda superior sagital+rektus+transvers, 1 olguda rektus+transvers ve 1 olguda transvers sinüste tromboz saptandı. Olguların 2'sinde subaraknoid kanama, 2'sinde lober hematom, Tinde subdural hematom ve Tinde hemorajik infarkt eşlik etmekte idi. Etyolojik incelemede 3'ü postpartum dönemde, Ti ise gebe idi. Olguların Tinde protein C eksikliği, Tinde aktive protein C rezistansı, Tinde AT III eksikliği, Tinde anti SUE pozitifliği, Tinde antikardiyolipin antikor pozitifliği ve Tinde trombositoz saptandı. 4 olguda etyolojik neden bulunamazken, lober hematomu olan 1 hasta erken dönemde kaybedildiği için detaylı inceleme yapılamadı. Olguların 9'unda tam, 3'ünde tama yakın düzelme olup, 2'sinde görme kaybı gelişmiştir. Yorum: SVT yüksek klinik şüphecilik gerektiren bir durumdur. Erken tedavi, ölüm ve ağır dizabilite riskini azaltabildiğinden, erken tanı çok önem taşımaktadır.
