Skull base erosion in chronic renal failure: Report of two cases

Abstract
Abstract Objective: Implication of the chronic renal failure in the etiology of the spontaneous rhinorrhea. Presentations of the cases and interventions: Spontaneous cerebrospinal fluid leakage was diagnosed in two patients with chronic renal failure. Anterior fossa repair was performed in the first case because of the enlarged foramina of the lamina cribrosa on cranial computed tomography. Sphenoid sinus was obliterated through the transsphenoidal route because of the suspected cerebrospinal fluid collection inside the sphenoid sinus in the second case. No cerebrospinal fluid leakage was found on postoperative one year follow up in both cases. Conclusion: We concluded the renal osteodystrophy associated to the chronic renal failure caused to skull base erosion and spontaneous rhinorrhea in the patients.
Özet
Amaç: Spontan rinore etyolojisinde kronik böbrek yetmezliğinin gösterilmesi. Olguların sunumu ve uygulanan girişim: Kronik böbrek yetmezliği olan iki olguda spontan beyin omurilik sıvısı kaçağı saptandı. Birinci olguda bilgisayarlı beyin tomografide lamina kribroza deliklerinin genişlemiş olarak görülmesi üzerine anterior fossa onarımı, ikinci olguda sfenoid sinüs içerisinde şüpheli beyin omurilik sıvısı kolleksiyonu görüntüsü nedeni ile transsfenoidal girişimle sfenoid sinüs obliterasyonu uygulandı. Ameliyat sonrası 1 yıllık izlemde her iki olguda da rinore görülmedi. Sonuç: Olgularımızda kronik böbrek yetmezliğinin seyri sırasında ortaya çıkan renal osteodistrofi'nin yol açtığı kafa tabanı erozyonuna bağlı olarak spontan rinore oluştuğu sonucuna varıldı.
