CA 125 Elevation Due to Minimal Change Disease

Abstract
Minimal change disease is a glomerular disease that can be primary or secondary and accounts for %1015 percent of adult nephrotic syndrome cases. Measurement of the serum concentration of the CA 125 glycoprotein antigen is the most widely used biochemical method of screening for ovarian cancer but the specificity of this method is limited. Mean CA 125 levels further vary with ethnicity and smoking status and increase with age. CA 125 levels are elevated in approximately 1 percent of healthy women and fluctuate during the menstrual cycle. Levels of CA 125 increase in a variety of conditions other than ovarian pathologies. In our case, we determined the reason of the approximately thirty times elevated CA 125 levels as the nephrotic syndrome We wanted to mention that even at these high levels, one should consider that the reason of the elevation of CA 125 may be nephrotic syndrome, not a malignant condition
Özet
Minimal değişiklik hastalığı, primer veya sekonder olarak gelişebilen,erişkin dönem nefrotik sendrom olgularının %10-15'ine neden olan glomerüler bir hastalıktır. CA 125 glikoprotein antijeninin serum konsantrasyonunun ölçümü, over kanserleri için en sık kullanılan, ancak spesivitesi sınırlı bir biyokimyasal takip yöntemidir. Ortalama CA 125 seviyeleri, etnisite ve sigara içme durumuna göre değişiklik gösterir ve yaşla birlikte artar. Seviyeleri, sağlıklı kadınların yaklaşık %1'inde yüksek saptanabilmekte, menstrüel siklus boyunca dalgalanma göstermekte, over patolojileri dışında birçok durumda da yüksek saptanabilmektedir. Olgumuzda, 19 yaşında kadın hastada yaklaşık otuz kat kadar artmış CA 125 yüksekliğinin nedeni olarak nefrotik sendrom saptandı. Ayırıcı tanılar arasında, bu kadar yüksek değerlerde dahi nefrotik sendromun akılda tutulması gerektiğini vurgulamayı amaçladık
