Antimicrobial Susceptibility and Distribution of Stenotrophomonas maltophilia Strains Isolated from Various Clinical Specimens in Five Years

Abstract

Aim: Currently the isolation rate of Stenotrophomonas maltophilia which is an opportunistic nosocomial pathogen, is increasing worldwide. It is commonly found in nature and also it can be recovered from oropharynx and sputum in adults. When nosocomial pneumonia with S. maltophilia is associated with obstruction and bacteremia, mortality rate increases. S. maltophilia is usually resistant to most of the ?-lactam antibiotics, ?-lactam inhibitors and aminoglycosides. Since most of the S. maltophilia strains are susceptible to trimethoprim/sulfamethoxazole, this antibiotic is widely used in the treatment. The aim of this study was to determine the susceptibility rates of S. maltophilia strains isolated in our hospital. Materials and Methods: This study was conducted in the Medical Microbiology Laboratory of Konya Training and Research Hospital, Turkey between 1 January2009 and 31 December 2013. Bacterial identification and antimicrobial susceptibility testing was performed by using conventional methods and Phoenix®automated system (Becton Dickinson Sparks, USA).Results: In this study, 121 S. maltophilia strains isolated from various clinical materials in our hospital were evaluated for antimicrobial susceptibility. In 2009, 13 S. maltophilia strains, in 2010 and 2011, 23, in 2012, 28 and in 2013 34 strains were isolated. Most of the strains were isolated from the intensive care units (51.9%), followed by inpatient clinics (43.4%) and outpatient clinics (4.6%). Most of the S. maltophilia strains were isolated from the respiratory samples such as sputum, pleural fluid and bronchoalveolar lavage. The highest rate of susceptibility was achieved with trimethoprim-sulfamethoxazole (84.8%). Susceptibility rates estimated for levofloxacine and ceftazidime were 76.2% and 22.3%, respectively. Conclusion: As a conclusion, maintenance of therapy for S. maltophilia infections should be planned based on the results of the antimicrobial susceptibility tests, instead of empirical treatment

Amaç: Stenotrophomonas maltophilia günümüzde gittikçe daha sık izole edilen, fırsatçı bir nozokomiyal enfeksiyon etkenidir. Doğada yaygın olarak bulunduğu gibi, erişkinlerin orofarinks ve balgamlarından sıklıkla izole edilebilir. Nozokomiyal Stenotrophomonas maltophilia pnömonisi özellikle obstrüksiyon ve bakteremi ile beraber olduğunda yüksek mortalite ile birliktedir. Stenotrophomonas maltophilia, çoğu ?-laktam, ?-laktam inhibitörleri ve aminoglikozitlere dirençlidir. Düşük direnç görülmesinden dolayı enfeksiyonlarının tedavisinde trimetoprim sulfometoksazol yaygın olarak kullanılmaktadır. Bu çalışma, hastanemizde izole edilen S. maltophilia suşlarının duyarlılık oranlarını saptamak ve sonraki çalışmalara katkı sağlamak amacıyla planlanmıştır. Gereç ve Yöntem: Çalışma, Konya Eğitim ve Araştırma Hastanesi Mikrobiyoloji Laboratuvarı'na 1 Ocak 2009 - 31 Aralık 2013 tarihleri arasında gönderilen örneklerden retrospektif olarak yapılmıştır. Bakterilerin tanımlanması ve antibiyotik duyarlılıkları Phoenix (Becton Dickinson, Sparks, ABD) tam otomatik tanımlama sistemi ve klasik yöntemlerle yapılmıştır.Bulgular: Laboratuvarımıza gönderilen çeşitli materyallerden izole edilmiş 121 S. maltophilia suşu çalışmaya alınmıştır. 2009 yılında 13, 2010 yılında 23, 2011 yılında 23, 2012 yılında 28 ve 2013 yılında 34 adet suş izole edilmiştir. En fazla sayıda suş yoğun bakım ünitelerinden izole edilirken (67; %51.9), bunu servis hastaları (56; %43.4) ve poliklinik hastalarından izole edilen suş sayıları (6; %4.6) takip etmiştir. S. maltophilia suşları en çok balgam, pevral sıvı ve bronkoalveolar lavaj gibi solunum sistemi örneklerinden elde edilirken, en az kulak akıntısında üretilmiştir. En duyarlı olduğu antimikrobiyal ajanın trimethoprim sulfametoksazol (%84.8) olduğu saptanmıştır. Levofloksasin ve seftazidim'e duyarlılık %76.2 ve %22.3'tür.Sonuç: Sonuç olarak, S. maltophilia enfeksiyonlarında ampirik tedavilerin uygulanmaması ve antibiyogram sonuçlarına göre tedavinin düzenlenmesi önemlidir
