In-vitro Activity of Daptomycin Against Enterococcus strains Isolated from Blood Cultures

Abstract
Objective: In recent years, increasing antimicrobial resistance in Enterococcus strains cause important problems especially in nosocomial infection all over the world. The effectiveness of daptomycin which is a cell wall-inhibiting agent with a spectrum of activity limited to gram-positive bacteria, has been demonstrated to have activity also against resistant enterococcus strains. The aim of this study is to identify the species and determine daptomycin susceptibility of enterococci isolated from blood cultures. Materials and Methods: In our study, a total of 64 Enterococcus strains isolated from blood culture samples of hospitalized patients between May 2011 to January 2014, were evaluated. The strains were identified at species level by API-20 STREP (BioMérieux, France) and/or automated VITEK 2 system (BioMérieux, France). Daptomycin susceptibilities were determined by E-test (AB Biodisk, Solna, Sveden) according to the CLSI (Clinical and Laboratory Standards Institute) guidelines. Susceptibilities to the other antibiotics were determined by KirbyBauer disc diffusion method according to CLSI guidelines. Results: Sixty-four strains isolated from blood cultures of different patients were identified at species level. Forty (62.5%) strains were identified as Enterococcus faecium, and 24 (37.5%) as Enterococcus faecalis. All of the isolates were susceptible to daptomycin and MIC range was 0.16-4 ?g/ml. MIC50 and MIC90 values for E. faecium, and E. faecalis were 1.5-3 ?g/ml, and 0.19-1 ?g/m, respectively. Three of the E. faecium isolates were resistant to vancomycin. Daptomycin MIC values for these two strains were 3 ?g/ml and 2 ?g/ml, respectively.Conclusion: Daptomycin exhibited in-vitro effectiveness against enterococci including vancomycin-resistant enterocci strains. Daptomycin may be thought to be a good alternative for the treatment of resistant enterococcal infections.

Amaç: Son yıllarda tüm dünyada enterokoklarda artan antimikrobiyal direnç, özellikle hastane kaynaklı enfeksiyonlarda önemli sorunlara neden olmaktadır. Hücre duvarı inhibitörü olan daptomisinin etkinliği gram pozitif bakterilerle sınırlı olup, dirençli enterokok suşlarında oldukça etkilidir. Bu çalışmanın amacı, kan kültürlerinden izole edilen enterokokların türlerini ve bu suşlar üzerine daptomisinin in-vitro etkinliğini saptamaktır. Gereç ve Yöntem: Çalışmamızda, Mayıs 2011 - Ocak 2014 tarihleri arasında hastanede yatmakta olan hastaların kan kültürü örneklerinden izole edilen toplam 64 enterokok suşu incelenmiştir. Bakterilerin tür düzeyinde tanımlanması API-20 STREP (BioMérieux, Fransa) ve/veya Vitek 2 (BioMérieux, Fransa) tam otomatik bakteri tanımlama sistemi ile yapılmıştır. Daptomisin duyarlılıkları CLSI (Clinical and Laboratory Standards Institute) önerileri doğrultusunda E-test (AB Biodisk, Solna, İsveç) metoduyla belirlenmiştir. İzole edilen suşların diğer antibiyotiklere duyarlılıkları CLSI önerileri doğrultusunda Kirby Bauer disk difüzyon yöntemi ile araştırılmıştır. Bulgular: Farklı hastalara ait kan kültürü örneklerinden izole edilen 64 enterokok cinsi bakteri tür düzeyinde tanımlanmıştır. Bu bakterilerin 40'ı (%62.5) Enterococcus faecium, 24'ü (%37.5) ise Enterococcus faecalis olarak tanımlanmıştır. Tüm suşlar daptomisine duyarlıydı ve MİK aralığı 0.016-4 ?g/ml olarak saptandı. MİK50 ve MİK90 değerleri E. faecium için 1.5-3 ?g/ml, E. faecalis için 0.19-1 ?g/ml olarak saptandı. E. faecium suşlarından 3'ü vankomisine dirençli idi. Bu suşlardan ikisi için MİK değeri 3 ?g/ml ve biri için 2 ?g/ml idi.Sonuç: Daptomisinin vankomisin dirençli enterokoklar da dâhil olmak üzere enterokok suşlarına karşı in-vitro olarak etkili olduğu saptanmıştır. Dirençli enterokokal enfeksiyonların tedavisinde daptomisinin iyi bir alternatif olabileceği düşünülmektedir.
