Co-infection with Campylobacter jejuni Subtypes in Patients with Acute Gastroenteritis

Abstract

Objective: In this study, it was aimed to evaluate the co-infection detected in three cases with gastroenteritis associated with different subtypes of C. jejuni.Materials and Methods: Phenotypic and molecular tests were used for the identification of isolates obtained from stool samples of three patients. Antibiotic susceptibility was evaluated by disc diffusion method. Ampicillin, amikacin, cefotaxime, tetracycline, piperacillin/tazobactam, erythromycin clindamycin, nalidixic acid, ciprofloxacin and levofloxacin were used in the antibiotic susceptibility testing. Results were evaluated according to the CLSI criteria. ERIC-PCR was used for the molecular typing of the isolates.Results: As a result of cultural evaluation of the samples belonging to 3 patients with gastroenteritis, all isolates grown on mCCD agar were identified as C. jejuni by using conventional and molecular methods. It is revealed that the antibiotic susceptibility patterns and ERIC-PCR band profiles of the two isolates recovered from each sample were different. Co-infection rate in this study was determined to be 1.7%.Conclusion: To our knowledge, this is the first scientific study conducted in Turkey to define co-infections caused by C. jejuni subtypes. It was concluded that it would be beneficial to examine and define all of the different colony types in petri dishes and to determine the antibiotic susceptibilities of all the isolates to guide the antimicrobial therapy accordingly in examining fecal cultures for Campylobacter species in clinical microbiology laboratories.

Özet

Amaç: Bu çalışmada, C. jejuni'nin farklı alt tiplerine bağlı üç gastroenterit olgusunda saptanan koenfeksiyonun değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Üç hastaya ait fekal örnekten elde edilen izolatların tanımlanmasında fenotipik ve moleküler testlerden yararlanılmıştır. Antibiyotik duyarlılığı, disk difüzyon yöntemi ile değerlendirilmiştir. Testte ampisilin, amikasin, sefotaksim, tetrasiklin, piperasilintazobaktam, eritromisin, klindamisin, nalidiksik asit, siprofloksasin ve levofloksasin antibiyotik diskleri kullanılmıştır. Sonuçlar CLSI kriterlerine göre değerlendirilmiştir. İzolatların moleküler tiplendirmesi için ERICPCR'den yararlanılmıştır.Bulgular: Gastroenteritli 3 hastaya ait örneğin kültürel değerlendirmesi sonucunda mCCD agarda üreyen izolatların tümü konvansiyonel ve moleküler yöntemler ile C. jejuni olarak tanımlanmışlardır. Her örneğe ait 2 izolatın antibiyotik duyarlılık paternlerinin ve ERIC-PCR'de elde edilen bant profillerinin farklılık gösterdiği saptanmıştır. Araştırmadaki koenfeksiyon oranı %1.7 olarak belirlenmiştir.Sonuç: Bilgilerimize göre bu araştırma, C. jejuni alt tipleri tarafından meydana getirilen koenfeksiyonu tanımlayan ülkemizde yapılmış ilk çalışmadır. Klinik mikrobiyoloji laboratuvarları'nda fekal kültürlerin kampilobakterler yönünden değerlendirilmesinde, petrilerdeki farklı koloni tiplerinin tümünün göz önünde bulundurularak tanımlanması, antibiyotik duyarlılık testlerinin yapılması ve klinik tedavinin buna göre yönlendirilmesinin yararlı olacağı sonucuna varılmıştır.
