Investigation of West Nile Virus in Cerebrospinal Fluid and Blood Samples of Patients with Initial Diagnosis of Central Nervous System Infection and Comparison of Results

Abstract

Objective: West Nile Virus (WNV) was firstly isolated in1937 in West Nile area of Uganda. This virus is an enveloped,45-50 nm sized RNA virus with icosahedral symmetry that isclassified in the Japanese encephalitis virus serocomplex ofgenus flavivirus of the family flaviviridae. The aim of ourstudy is to show WNV specific antibodies or viral nucleicacid in blood and/or cerebrospinal fluid (CSF) o fthepatients with initial diagnosis of CNS infections.Materials and Methods: Routine blood and CSF sampleswithout any identifiable pathogenic microorganism of thepatients who were treated with the initial diagnosis of centralnervous system infection which were sent to the hospital'scentral laboratory, were collected. Ninety-four samples of 47patients (47 CSF and 47 blood) were examined with nodistinction of age, gender, and service. WNV were examinedby ELISA and RT-PCR methods in these samples.Results: Average age of the patients included in this study was19 years. CSF samples of the patients showed pleocytosis(average 155 cells), moderately high protein levels (mean 89mg/dl) and mildly low glucose levels (mean 53 mg/dl). In thisstudy, only in one patient's blood sample suspect WNV IgMpositivity was detected by ELISA. In all other samples WNVIgM and IgG antibodies were not detected. The CSF andblood samples of the suspected WNV IgM positive sampleswere found to be negative by RT-PCR method. Similarly, allsamples were tested by RT-PCR method and no RNA relatedto WNV was detected.Conclusion: In our study, suspect positive result wasconsidered to be related with cross reactivity that can bedetected in ELISA method. BNV seronegativity mostlyresulted from the low mean age level (mean: 19) of thepatients included in the study. Also according to the datawe obtained in our study with PCR and ELISA, WNV borneCNS infections were not encountered in our region.Depending on the ecological conditions more comprehensivestudies will be needed. We hope our work is of benefit forthe WNV database in our country.

Özet

Amaç: Batı Nil virüsü (BNV); ilk defa 1937 yılında Uganda'nın Batı Nil kesiminde izole edilmiştir. Bu virüs Flaviviridae ailesi, Flavivirüs cinsi ve Japon ensefaliti virüsü serokompleksi içerisinde sınıflandırılan, 45-50 nm büyüklüğünde, zarflı ve ikozahedral yapılı bir RNA virüsüdür. Çalışmamızın amacı, SSS enfeksiyonu ön tanılı hastaların kan ve/veya beyin-omurilik sıvısı (BOS)'nda BNV'ye özgü antikorların veya viral nükleik asidin gösterilmesidir. Gereç ve Yöntem: Santral Sinir Sistemi enfeksiyonu ön tanısıyla tedavi gören ve herhangi bir patojen mikroorganizma tespit edilemeyen hastaların, hastanenin merkez laboratuvarına gönderilen rutin kan ve BOS örnekleri toplandı. Yaş, cinsiyet ve gönderilen servis ayırımı yapılmaksızın kırk yedi hastanın 94 örneği (47'si BOS ve 47'si kan) değerlendirilmeye alındı. Bu örneklerde ELISA ve RT-PCR yöntemleriyle BNV araştırıldı.Bulgular: Çalışmaya alınan hastaların yaş ortalaması 19 idi. Bu çalışmanın BOS örneklerinde; pleositoz (ortalama 155 hücre), orta derece protein artışı (ortalama 89 mg/dL) ve hafif glukoz (ortalama 53 mg/dL) düşüklüğü tespit edilmiştir. Bu çalışmada, yalnızca bir hastanın kan örneğinde ELISA yöntemi ile BNV IgM şüpheli pozitif saptanmıştır. Diğer örneklerde BNV IgG ve IgM antikoru negatif olarak saptanmıştır. Bu şüpheli örneğin BOS'u ve kanı gerçek zamanlı RT-PCR yöntemi ile negatif bulunmuştur. Benzer şekilde tüm örnekler gerçek zamanlı RT-PCR yöntemi ile de çalışılmış, BNV'ye ait RNA tespit edilememiştir. Sonuç: Çalışmamızda şüpheli değer ELISA yönteminde görülebilen çapraz reaksiyonlara bağlanmıştır. BNV seronegatifliği büyük ölçüde çalışmaya alınan hastaların yaş ortalamasının (ortalama: 19) düşük olmasından kaynaklanmıştır. Ayrıca PCR ve ELISA'yı beraber dikkate aldığımızda çalışmamızda elde ettiğimiz verilere göre, bölgemizde BNV kaynaklı SSS enfeksiyonuna rastlanmamıştır. Ekolojik koşullara bağımlı olarak daha kapsamlı çalışmalara gereksinim olacaktır. Çalışmamızın bu konuda ülkemiz BNV veri tabanına yarar sağlamasını ümit etmekteyiz.
