Comparison of the Results of BD GeneOhm VanR Assay with Those of the Culture for Screening of VancomycinResistant Enterococci

Abstract
Objective: Culture-based vancomycin resistant enterococci(VRE) identification methods generally require 24-72hours. Rapid isolation of VRE colonised patients is crucialto prevent colonisation of other patients and propableinfections. The aim of this study is to compare the results ofBD GeneOhm VanR test which identifies VRE in a shorttime with the gold standard which is VRE culture.Material and Methods: A total of 704 samples in the studywere cultivated on VRE agar. At the same time, all perirectalswap samples were evaluated with real-time PCR.Results: Among the evaluated samples, 121 (17.2%) werepositive with VRE agar and 162 (23.0%) were positive withreal-time PCR. The number of samples which were positivewith both diagnostic methods were 118. Of the samples, 44were found to be positive only with real-time PCR and 3with culture methods. The number of samples negative withboth methods were 539 (76.6%). All VRE positive samplesexcept 3 were Enterococcus faecium carrying VanAphenotype, and the 3 samples mentioned were identified asE. faecalis carrying VanB phenotype with both culture andreal-time PCR. According to these results; sensitivity,specificity, positive predictive (PPV) and negative predictivevalues (NPV) of BD GeneOhm VanR test compared to VREculture were 98, 92, 73, and 99%, respectively.Conclusion: Culture-based methods and nucleic acidamplification tests require 24-72 hours for VRE identification.BD GeneOhm VanR test is a simple diagnostic method thatis directly carried out on the sample and takes less than 3.5hours to obtain results. On the other hand, it gives repeatableresults in patient samples and in different laboratories, andit has high PPV and NPV. Its other advantage is theidentification of the VRE-resistance gene.

Amaç: Kültür bazlı vankomisin dirençli enterokok (VRE) identifikasyon yöntemleri genellikle 24-72 saat gerektirmektedir. VRE kolonizasyonu saptanan hastaların hızla izole edilmesi, diğer hastaların kolonize olmalarını engellemek ve olası enfeksiyonların oluşumunu önlemesi açısından değerlidir. Bu çalışmanın amacı, VRE'yi daha kısa sürede saptayan BD GeneOhm VanR testinin sonuçlarının altın standart yöntem olan VRE kültürü ile karşılaştırılmasıdır.Gereç ve Yöntem: Çalışmamızda yer alan toplam 704 perirektal sürüntü örneğinin VRE agara ekimi yapılmıştır. Eşzamanlı olarak tüm perirektal sürüntü örnekleri Realtime PCR yöntemi ile çalışılmıştır.Bulgular: Değerlendirilen örneklerden 121'i (%17.2) VRE agar ile 162'si (%23.0) Real-time PCR yöntemi ile pozitif bulunmuştur. Yöntemlerinin her ikisi ile pozitif çıkan örnek sayısı 118'dir. Kırk dört örnek yalnızca Real-time PCR ile 3 örnek ise yalnızca kültür yöntemiyle pozitif bulunmuştur. Her iki yöntemle negatif bulunan örnek sayısı 539 (%76.6)'dur. VRE pozitif saptanan örneklerden 3 tanesi haricinde tümü, VanA fenotipi taşıyan Enterococcus faecium olarak, sözü edilen 3 örnek, hem kültür hem de Real-Time PCR yöntemi ile VanB fenotipi taşıyan Enterococcus faecalis olarak tanımlanmıştır. Bu sonuçlara göre, BD GeneOhm VanR testinin VRE kültürü ile karşılaştırılarak belirlenen duyarlılık, özgüllük, pozitif (PPD) ve negatif prediktif değerleri (NPD) sırasıyla %98, %92, %73 ve %99 olarak saptanmıştır.Sonuç: VRE belirlenmesi için kullanılan kültür bazlı yöntemler ve nükleik asit amplifikasyon testleri genellikle 24-72 saat gerektirmektedir. BD GeneOhm VanR testi doğrudan örnekten çalışılan basit bir yöntem olup, sonuç alma aşamasına kadar geçen toplam süre 3.5 saatten daha kısadır. Bunun yanında farklı laboratuvarlarda ve hasta popülasyonlarında yinelenebilir sonuçlar veren, yüksek PPD ve NPD olan bir testtir. VRE direnç genini de belirliyor olması da diğer bir avantajıdır
