Evaluation of Microarray and Lysis Filtration Combined MALDI TOF MS Procedures for the Identification of Gram Negative Bacteremia

Abstract
Objective: Each hour of delay in antibiotics administration increases mortality in sepsis. The aim of this study was to decrease the bacteria identification time to initiate appropriate antibiotic treatment as early as possible. Method: Tests were applied to 39 Gram negative bacteria isolated from blood cultures sent to our laboratory from intensive care units between November 2015- February 2016. The results of bacterial identification tested on both microarray and LFM methods were compared. Results: In the comparison of MALDI-TOF MS after sub-culture, MALDI-TOF after lysis centrifugation and microarray methods, sensitivity was determined as 82% (32/39) in LFM and as 87.1% (34/39) in the microarray method. All three methods had a concordance of 76.9% (30/39). Most common species identified in this study were Acinetobacter spp., Klebsiella spp. and Escherichia spp., and their Cohen’s Kappa coefficients for LFM and post-subculture MALDI were calculated as 0.715, 0.843, and 0.938, respectively. In addition, their BC-GN microarray and post-subculture MALDI concordance rates calculated with Cohen’s Kappa were 0.935, 0.753 and 0.938, respectively. Both methods showed good correlations with the post-culture MALDI method. Conclusion: Lysis centrifugation and microarray platforms decrease the identification time in blood culture processing successfully. Results of this study suggest that for the laboratories with MALDI-TOF mass spectrophotometer, the lysis filtration method is a fast and cost-effective method that may be suitable for routine procedures.

Amaç: Sepsis olgularında yanlış antimikrobiyal tedavi alan veya tedavi almayan olgularda ölüm oranları kaybedilen her saat başına artış göstermektedir. Bu çalışmada, etkenin konvansiyonel kültür yöntemine oranla daha hızlı tanımlanabilmesi amacıyla konvansiyonel kan kültürü/MALDI TOF yöntemi ile lizis filtrasyon sonrası MALDI TOF MS (LFM) ve mikroarray yönteminin karşılaştırılması amaçlanmıştır. Yöntem: Kasım 2015-Şubat 2016 tarihleri arasında Ege Üniversitesi, Tıp Fakültesi Hastanesi, Erişkin Yoğun Bakım ünitelerinde izlenen hastalardan alınan kan kültürü örneklerindeki 39 Gram negatif etkene mikroarray ve LFM işlemi uygulanıp, ardından MALDI TOF sistemi ile çalışıldı. Bakteri kolonisinden yapılan MALDI TOF tür düzeyinde tanımlama ile lizis filtrasyon sonrası uygulanan MALDI TOF ve mikroarray test yöntemlerinin sonuçları karşılaştırıldı. Bulgular: Kan kültürü cihazındaki pozitif sinyal süreleri en kısa yedi saat 55 dakika, en uzun 31 saat 22 dakika, ortalama pozitiflik süresi 12 saat 55 dakika olarak belirlendi. Kültür sonrası MALDI TOF identifikasyonu ile LFM identifikasyonun duyarlılığı %82 (32/39), mikroarray yönteminin ise duyarlılığı %87.1 olarak hesaplandı. Lizis filtrasyon sonrası uygulanan MALDI TOF ile mikroarray yöntemlerinin birbirleri ile benzerliği %79.4 (31/39), her üç yöntemin birlikte tutarlı sonuç verdiği örneklerin oranı ise %76.9 (30/39) olarak hesaplandı. En sık saptanan üç etken Klebsiella spp, Acinetobacter spp. ve Escherichia spp. kültür sonrası MALDI TOF ve LFM yöntemi ile uyumu Cohen’in kappa katsayısı sırası ile 0.715, 0.843, 0.938; mikroarray yöntemi ile uyumu 0.935, 0.753, 0.938 olarak hesaplandı ve her üç bakteri için de yüksek uyumluluk elde edildi. Sonuç: Elde edilen verilere dayanılarak, hem lizis filtrasyon hem de mikroarray platformunun sepsis tanısını kısaltmaları açısından yararlı olduğu düşünülmüştür. Lizis filtrasyon yöntemi maliyet etkinlik açısından önde bulunmuştur.
