Long-term clinical and angiographic follow-up results of multi-link coronary stent implantations

Abstract
The ACS Multi-link (ML) intracoronary stent is a second generation, balloon expandable stainless steel stent designed to avoid the negative features of currently available stents. The largest amount of information available on the long term outcome of coronary stenting is based on the use of Palmaz-Schatz stent. Less data exist on long-term follow-up results of Multi-link coronary stent implantations. We present the long-term (>3 years) clinical and angiographic follow-up results of the ACS Multi-link coronary stents implanted in our institution. From May 1996 to December 1997 a total number of 125 patients underwent 133 coronary ML stent implantations. Stented vessels were: 49% LAD, 31% RCA, 20% Cx coronary artery. Indications fot stent implantation were elective in 64%, suboptimal result in 26%, bailout in 10% of patients. The mean reference diameter of stented vessels was 3.2 ± 0.2 mm. The mean percentage stenosis was 80±11% and 3±5 % before and after stent implantation, respectively. Long-term clinical follow-up was completed in 75% (80 male, mean age 53±10) of the patients (either by interview or phone), and angiographic follow-up (37±12 months) was completed in 58% of the patients. There was no baseline clinical and angiographic differences between angiographically checked and the remaining patients. Angiographic restenosis (>50% diameter stenosis) was detected in 22% of stents. Target lesion revascularisation was 12%, non-target lesion revascularisation was 14% in angiographically followed pts. During follow-up period death and new MI occurred in 12% and 6% of patients, respectively, and survival rate was 88%. Our study provides long-term follow-up results of intracoronary Multi-link stent implantations for native coronary artery lesions. Our data show that clinical and angiographic benefit of ML stents is comparable to the first generation stents especially to the Palmaz-Schatz stents of which results have been reported previously. An important rate of non target lesion revascularisation occurs during follow-up period.

Koroner stent uygulamalarının uzun dönem sonuçlarına ait verilerin büyük çoğunluğu birinci jenerasyon stentlere ve özellikle Palmaz-Schatz stent uygulamalarına aittir. Daha az sayıdaki çalışmada ikinci jenerasyon stentler içerisinde yer alan MULTİLİNK (ML) stentin orta dönem (6. ay-1.yıl) sonuçları incelenmekle beraber bu stentin uzun dönem (>3 yıl) takip sonuçlarına ait veriler bulunmamaktadır. Çalışmamızın amacı, kliniğimizde intrakoroner ML stent implantasyonu yapılmış olguların uzun dönem (>3 yıl) klinik ve anjiyografik sonuçlarının incelenmesidir. Mayıs 1996-Aralık 1997 tarihleri arasında 125 olguya toplam 133 ML stent yerleştirildi. Stent endikasyonlarının, %10'u bail-out, %26'sı suboptimal sonuç, %64'ü elektif stent şeklindeydi. Hedef lezyonların %49'u LAD, %31'i RCA, %20'si CX arterde lokalizeydi. Referans damar çapı 3.2 ± 0.2 mm, işlem öncesi lezyon yüzdesi %80 ± 11, işlem sonrası lezyon yüzdesi % 3±5 idi. Belirtilen tarihler arasında kliniğimizde ML stent yerleştirilmiş olguların %75'ine (94/125) (80 erkek, 14 kadın, yaş ort. 53 ± 10) ulaşılabildi. Bu olgulardan 73'üne stent uygulamasından ortalama 37 ± 12 ay sonra kontrol koroner anjiyografi yapıldı. Anjiyografik takibi yapılabilen olgular tüm olguların %58'ini (73/125) oluşturmaktaydı ve temel özellikler açısından anjiyografik takibi yapılamayan olgularla farklılık göstermemekteydiler. Uzun dönem kontrol anjiyografisi yapılan stentlerin %22'sinde (17/78) restenoz veya total okluzyon gözlendi. Anjiyografisi yapılan olgularda hedef lezyon revaskülarizasyon (HLR) oranı %12 (9/73), hedef lezyon dışı revaskülarizasyon (HLDR) oranı %14 (10/73), yeni MI %6, ölüm %12 olarak gözlendi. Buna göre ölüm oranı %12, sürvi oranı % 88 olarak saptandı. Çalışmamız ML stent uygulanan olgularda gözlenen uzun dönem klinik ve anjiyografik izleme sonuçlarını vermektedir. Verilerimiz ML stentlerin uzun dönem takip sonuçlarının gerek aynı cins stentlerle yapılmış orta dönem takip sonuçlarına göre gerekse birinci jenerasyon stentlerden Palmaz-Schatz stentlerin uzun dönemdeki takip sonuçlarına göre benzer veya daha iyi olduğunu göstermektedir. Olgularımızda gözlenen yeni koroner lezyon ve HLDR oranları ateroskleroz progresyonunun yavaşlatılmasına yönelik girişimlerin önemini bir kez daha ortaya koymaktadır.
