Peripheral self-expandable stent catheter fracture: Retrieval of two different components with two different modalities

Abstract

A 58-year-old man whose right superficial femo- ral artery had been stented 2 weeks previously presentedat the emergency service complaining of right leg pain of 2days duration. Angiography revealed a radiopaque foreignbody distal to the stent. It was removed with a snare. Due tosudden disappearance of the popliteal artery pulse seventy- two hours after the procedure, an urgent control angiogramwas performed, which showed a large thrombus occludingthe superficial femoral artery. Surgery revealed a secondlarger radiolucent foreign body causing the thrombus. Thiswas recognized as the distal tip and shaft of the peripheralself-expandable stent catheter used in the initial procedure.This report discusses preventive measures to be takenagainst this complication.

Özet

İki hafta önce sağ yüzeyel femoral arterine stent ta- kılmış 58 yaşında erkek hasta, acil servise iki gündür olansağ bacak ağrısı ile başvurdu. Anjiyografide stent distalin- de radyoopak yabancı cisim görüldü ve snare yardımıylaçıkarıldı. İşlemden 72 saat sonra popliteal arter nabzınınaniden kaybolması nedeniyle acil periferik anjiyografi ya- pıldı ve yüzeyel femoral arterin yoğun trombüs ile tıkalıolduğu görüldü. Cerrahi girişim sonrasında trombüsün ne- deninin daha büyük ışın geçiren bir yabancı cisim olduğuanlaşıldı. Yabancı cisimlerin ilk işlemde kullanılan kendili- ğinden genişleyebilen stent kateterinin şaftı ve distal ucuolduğu anlaşıldı. Bu olgu sunumunda, bu komplikasyondankorunmanın yolları tartışıldı.
