Illusion or reality? How 3-dimensional optical coherence tomography overcomes the limitations of angiography: OCT-guided percutaneous coronary intervention of left main stem disease involving LAD/LCx bifurcation

Abstract
Angiography is still the workhorse imaging approachfor the vast majority of percutaneous coronary revascularization(PCR) cases. However, the limitations of angiographicguidance for coronary procedures have been wellestablished. This case report demonstrates how 3-dimensionaloptical coherence tomography (3D OCT) can changethe treatment strategy, which, had only the angiographyresults been considered, seemed to be straightforward. A67-year-old male patient presented with non-ST-elevationmyocardial infarction. A coronary angiogram revealed atight lesion of the proximal left anterior descending artery(LAD). Angiographically, the vessel ostium appeared not tobe involved. To clarify the disease border and determine theright stenting strategy, 3D OCT was performed. Measurementsrevealed heavy disease of the LAD, ranging from theostium to the left main coronary artery (LMCA). It was decidedto proceed with a provisional stenting strategy of theLMCA and the LAD. After postdilatation, the angiographyrevealed a good result for the LAD, but significant pinchingof the ostium of the LCx. The kissing balloon technique wasthen applied at the LAD/LCx bifurcation. The final OCT examinationdocumented a well-expanded stent without areasof malapposition and an open LCx ostium without significantnarrowing. Intracoronary images obtained using OCT addsignificant information to what is provided by angiographyalone, thereby improving the interpretation of angiographicimages and the planning of the PCR procedure.

Anjiyografi, perkütan koroner girişim (PKG) sırasında yaygın olarak kullanılan bir görüntüleme metodudur, fakat lezyonun gerçek boyutunu değerlendirmede sınırlı bilgiler verir. Bu olgu sunumu, anjiyografik görüntüleme sırasında basit olarak değerlendirilen bir lezyonda, 3-boyutlu optik koherens tomografinin (3D OCT) tedavi stratejisini nasıl değiştirdiğini göstermektedir. ST yükselmesiz miyokart enfarktüsü geçiren 67 yaşındaki erkek hastanın koroner anjiyografisinde proksimal sol ön inen arterde (LAD) ciddi darlık tespit edildi, fakat hastalığın ostiyuma kadar uzanmadığı görüldü. Hastalık sınırını net olarak belirlemek ve doğru girişim stratejisini belirlemek için, işlem öncesinde 3 boyutlu optik kohorens tomografisi yapıldı. Ölçümler, ostiyumdan başlayıp sol ana koroner artere (LMCA) kadar uzanan uzun bir LAD lezyonunu ortaya koydu. LMCA ve LAD’yi kapsayan provizyonel stent stratejisi ile ilerlemeye karar verildi. Postdilatasyondan sonra, anjiyografik olarak LAD’de iyi sonuç alınırken, LCx’in ostiyumunda belirgin bir daralma tespit edildi ve LAD/LCx bifürkasyona kissing balloon tekniği uygulandı. Nihai OCT işlemi, stentin iyi genişlemiş olduğunu ve LCx ostiyumunun açık olduğunu gösterdi. Optik koherens tomografisi ile elde edilen görüntüler, anjiyografinin tek başına sağladığı bilgilere önemli ölçüde katkıda bulunur ve böylece bize PKG stratejisini doğru bir şekilde planlama fırsatı verir.
