Gastrointestinal diseases in our outpatients with anemia

Abstract

It is very often for clinicians to give iron preparations without making differential diagnosis, after anemia is detected. Especially in patients with iron deficiency anemia, endoscopic and radiological procedures, which are very important to clarify the etiology, are thought as invasive methods. That is why, we aimed to evaluate differential diagnosis of our outpatients with anemia. Forty-seven patients with anemia (21 males, 26 females and mean age 50.5±9.4 years) were included into the study. Forty-two (89.3%) patients were shown to have iron deficiency and 5 (10.6%) of 47 patients had megaloblastic anemia due to vitamin B12 deficiency. Most of our patients had been given iron replacement therapy before they admitted to our hospital, but their complaints had persisted. We found out active hemorrhoid in 6 patients, atrophic gastritis in 5, benign gastric ulcer in 5, duodenal ulcer in 6, diverticulosis coli in 1, malabsorption due to postgastrectomy in 2, gastric cancer in 2 and colon cancer in 2 patients. We could define the etiology in 29 cases (61.7%). We could not find out the origin of bleeding in a patient with presence of occult blood in stool, even though angiographic, endoscopic procedures and radionuclide scanning were performed. In seventeen (36.1%) patients, lack of enough iron intake by diet was the etiology of iron deficiency. As a result, in patients with anemia, clinicians should make a very careful differential diagnosis by using endoscopic and other invasive procedures whenever it is needed

Pratik uygulamalarda anemi saptanması sonrası, genellikle ayırıcı tanıya gidilmeksizin demir replasmanına geçilmesi alışkanlığı yaygındır. Özellikle demir eksikliği anemisi olgularında gastrointestinal sistemin incelenmesinde kullanılan radyografik ve endoskopik girişimler pratisyen hekimlikte invaziv yöntemler olarak düşünülmektedir. Çalışmamızda gastroenteroloji polikliniğimize çeşitli yakınmalarla başvuran ve anemi saptanan olgularımız ayırıcı tanı yönünden değerlendirildi. Anemi saptanan 47 (21 erkek, 26 kadın; yaş ortalaması 50.5±9.4 yıl) olgu çalışmaya alındı. Kırkiki (%89.3) olguda demir eksikliği anemisi, 5 (%10.6) olguda ise vitamin B12 eksikliğine bağlı megaloblastik anemi saptandı. Olgularımızın 6'sında aktif hemoroid, 5'inde atrofik gastrit, 5'inde benign mide ülseri, 6'sında duodenal ülser, 1' inde kolonda yaygın divertiküler hastalık, 2' sinde post-gastrektomiye bağlı malabsorbsiyon, 2'sinde mide kanseri, 2'sinde kolon tümörü saptandı. Böylece 29 (%61.7) olgumuzda etiyoloji belirlendi. Bir olguda alt gastrointestinal sistemden kan kaybı belirlenmesi üzerine endoskopik girişimlere, anjiografi ve sintigrafiye rağmen odak saptanamadı. Onyedi (%36.1) olguda ise beslenme yetersizliği düşünüldü. Sonuç olarak, anemi saptanan her olgunun ayıncı tanısının, gereğinde invaziv olarak düşünülen endoskopik girişimler de dahil olmak üzere, tüm yöntemler kullanılarak yapılması gerektiğini önemle vurgularız.
