The correlation of CLO test reacting time and Helicobacter pylori density and severity of acute and chronic gastritis

Abstract
To determine whether the urease-based CLOtest can predict density of Helicobacter pylori (Hp) on antral biopsy and severity of acute and chronic gastritis, we evaluated antral biopsies obtained at endoscopy in 140 patients. The CLOtest was read at the end of first, fourth and twenty-fourth hours. Bacterial density was assessed on May Grunwald-modified Giemsa stain, and the severity of acute and chronic inflammation on hematoxylen and eosin graded from 0 to 3. Early CLOtest positivity (0-4 hours) was significantly associated with grade 2-3 Hp density (p= 0.03, p= 0.001, respectively). Correlation between"the time to a positive CLOtest and bacterial den¬sity was also good in the both acute and chronic gastritis. There was no sinificant association between early CLOtest positivity (0-4 hours) and grade 2-3 inflammation (p= 0,22) in acute gastritis, the same association in Tronic gastritis was weak (p= 0.052, Yates'correction). The time to a positive CLOtest result did not correlate with the degree of gastritis in the patients with acute gastritis. There was a weak correlation between the time to a positive CLOtest and the degree of gastritis. In conclusion, CLOtest predicts the bacterial density on the gastric mucosa but not the severity of the gastritis.

Bu cahsmada, CLOtestin reaksiyon verme siiresi ile gastrik biyopsideki Helicobacter pylori (Hp) yogunlugu ve gastritin siddeti arasmdaki iliskilerin ortaya gikanlmasi, amaclandi. galisma akut veya kronik gastritli 140 hastadayiirutiildii. CLOtest 1, 4 ve 24 saat bitiminde okundu. May Grunwald ve modifiye Giemsa ile boyanan doku kesitlerinde Hp arandi, hematoksilen ve eozin ile boyanan kesitlerde histolojik degerlendirme yaptldt. Hp dansitesi ve gastritisin §iddeti 0, 1, 2, 3 olarak derecelendirildi. CLOtestin 0-4 saattepozitiflesmesi (erken CLO+) ile doku kesitlerinde 2-3. derece Hp yogunlugu arasinda hem akut hem de kronik gastritlerde anlamh birlikte-lik bulundu (sirasiyla, p-0.03, p=0.001). CLOtestin pozitiflesme zamani ile bakteriyel yogunluk arasinda her iki gastritgrubunda da iyi derecede-korelasyon gozlendi. Erken CLO+ ile 2-3. derece gastrit arasinda akutgast-ritgrubunda anlamh iliski bulunamazken kronik gastrit grubunda zayif birliktelik bulunabildi (p= 0.052, Ya-tes dilzeltmesi). Akut gastrit grubunda CLOtestin pozitiflesme zamani ile gastritin siddeti arasinda korelasyon bulunamadt, ancak kronik gastrit grubunda zayif-orta derecede korelasyon tesbit edildi. Sonug olarak, CLO¬testin pozitiflesme siiresi mide mukozasimn Hp yogunlugu ileyakmdan iliskilidir, ancak inflamasyonun sid¬deti hakkmda bir on bilgi vermez.
