Tiroid fonksiyon bozukluklarında tedavi öncesi ve sonrası büyüme hormonu ve insülin benzeri büyüme faktörü-1 düzeyleri

Abstract
Objective: There is increasing evidence to suggest that thyroid hormones are intricately involved in the regulation of the growth hormone (GH)-insulin-like growth factor-1 (IGF-1) axis at a number of levels. Method: We investigated the effects of both hyper and hypothyroidism on GH and IGF-1 levels in 51 hyperthyroid and 39 hypothyroidpatients before and after treatment. Results: Mean GH concentration was decreased from a basal level of1.99+2.20 ng/ml to 1.83±2.37 ng/ml hyperthyroid patients (p> 0.05). Mean GH concentration was increased from a basal level of 1.46+2.16 ng/ml to 1. 78±2.52 ng/ml in hypothyroid patients (p> 0.05). Mean IGF-1 concentrations were increased in both hyper and hypothyroid subjects from basal levels of228.3±125.8 ng/ml and 149.4+125.8 ng/ml to 248.0 ±127.9 ng/ml and 175.7+121.3 ng/ml respectively. There were statistically significant differences when the levels of IGF-1 were compared between hyperthyroid and hypothyroid groups before and after treatment (p< 0.01, p< 0.05). But there was no statistically significant differences in each group when compared before and after treatment. Conclusion: We have confirmed that the hypothyroidism might cause the decrease in GH and IGF-1 levels. But there are complex relations between hyperthyroidism and GH-IGF-1 axis.

Amaç: Mevcut bulgular troid hormonunun, büyüme hormonu (GH) İnsülin benzeri büyüme faktörü-1 (IGF-1) aksım değişik aşamalarda etkilediğini göstermektedir. Bu çalışmada hiper ve hipotroidinin GH ve IGF-1 düzeylerine etkisi araştırılmıştır. Yöntem; GH ve IGF-1 düzeyleri 51 hipertroidi ve 39 hipotrodili hastada hem tedavi öncesi hem de tedavi sonrası ölçüldü ve değerler karşılaştırıldı. Bulgular: Ortalama GH düzeyleri tedavi öncesi ve sonrası sırası ile hipertroidili hastalarda 1.99±2.20 ng/ml ve 1.83±2.37 ng/ml, hipotroidili hastalarda 1.46±2.16 ng/ml ve 1.78±2.52 ng/ml idi (p> 0.05). Ortalama IGF-1 düzeyleri ise tedavi öncesi ve sonrası sırasıyla hipertroidili hastalarda 228.3±125.8 ng/ml ve 248.0±127.9 ng/ml, hipotroidili hastalarda 149.4±125.8 ng/ml ve 175.7±121.3 ng/ml idi. Hiper ve hipotroidili hasta gruplarında hem tedavi öncesi hem de tedavi sonrası değerlerin karşılaştırılmasında istatistiksel olarak anlamlı fark vardı (sırasıyla p< 0.01, p< 0.05). Fakat her bir grupta tedavi öncesi ve sonrası değerlerin karşılaştırılmasında anlamlı fark yoktu. Yorum; Bulgularımız hipotroidizmin GHveIGF-1 düzeylerini azalttığını hipertroidizmde ise GH-IGF-1 aksında kompleks bir etkileşme olduğunu göstermektedir.
