Pancoast tumour mimicking the symptoms of cervical spondylosis and shoulder periarthritis: Two cases report

Abstract

The initial symptom of Pancoast syndrome is shoulder pain, produced by neoplastic involvement of the brachial plexus, parietal pleura, endothoracic fascia, vertebral bodies, and first, second and third ribs. Pain can radiate up to the head and neck or down to the medial aspect of the scapula, axilla, anterior part of the chest, or ipsilateral arm, often along the distribution of the ulnar nerve. Treatment for presumed cervical osteoarthritis or bursitis of the shoulder is not uncommon, and in many series, a delay of 5 to 10 months is the correct diagnosis has been reported. We present two cases with Pancoast tumour as presumed periarthritis of the shoulder and cervical Spondylosis and were treated accordingly.

Özet

Pancoast sendromunun başlangıç belirtisi, neoplastik yayılımın brakial pleksus, paryetal plevra, endotorasik fasiya, vertebra cisimleri, 1, 2 ve 3- kaburgaların tutulması ile meydana gelen omuz ağrısıdır. Ağrı başa, boyuna veya skapulanın aşağı medial yüzüne, axilla, göğsün ön tarafına veya tek kola ve sıklıkla ulnar sinir dağılımınca yayılabilir. Literatürde 5-10 ay gibi bir süre servikal osteoartrit veya omuz bursiti düşünülerek tedavi edilmeye çalışılan Pancoast sendromuna rastlanmaktadır. Bu makalede omuz periartriti ve servikal spondilozis düşünülerek tedavi edilmeye çalışılan ancak daha sonra Pancoast tümörü tanısı konan bu iki vakayı sunmak istedik.
