The evaluation of snake bite cases in our clinic

Abstract
Objective and Method in this study we have evaluated the clinical symptoms, the toxic effects, the complicati-on and the treatments of snake bite in 18 cases prospectively. Results: Three (16.7%) of the cases were men while 15 (833%) were women; with mean age 41.4 (15-71 age), Twelve of the cases (66.7%) were bitten in tea-gardens while 6 of the cases (333%) in gardens or thickets bet-ween May-September at 10:00-12:00 or 15:00-18:00 o'clock. When the bitten area was considered, it was de-tected that 14 (77.8%) of the cases were bitten at a single foot, a single case (5.6%) was bitten at both feet and 3 of the cases (16. 7%) had a bite at their single hand. After the bite in the first hour nausea, vomiting, diarrhea hypotension, fainting sensation, panic and pain were reported by the patients First admission to a health center was in 2.5 hours (0.5-18 hours) of time, 6.5 hours (1-20 hours) was the mean time far the patients to admit to our clinic. At the first admission to a health center all the patients had steroid and antihistaminic tre-atment. We add antivenom to 7 (38.9%) of the cases and antibiotic to 14 (77.8%) of the cases far cellulites or throbophletis. Tetanus immunization were given in 15 (833%) of the cases. Ten (55.6%) of the cases progres-sively developed pitosis All of the cases recovered without sequels within 2-10 days (mean: 4 days). Conclusion: To our knowledge this is the first repon about snake bite in our country. in this repon the kinds of the snakes in our country and the treatments of snake bite were discussed. it has been emphasized that antivenom that we have today might be insufficient far the treatment.

Amaç: Zehirli yılan sokmaları hızlı tedavi gerektiren acil tıbbi bir durumdur ve tedavisi konusunda fikir birliği oluşmamıştır. Bölgemizde, tanmla uğraşın fazla olduğu kesimlerde olduğu gib rastlanmaktadır. Yöntem: 1994-2000 yıllan arasında kliniğimize başvuran 18 yılan sokma olgusu prospektif olarak değerlendirilmiş, klinik tabloları, toksik etkileri ve komplikasyonlan ile tedavi yaklaşımları gözden geçirilmiştir. Bulgular: Olguların 3 (%16.7)'ü erkek, 15 (%83-3)'i kadın, yaş ortalamaları 41.4 (15-71 yaş) olarak belirlenmiştir. Olguların 12 (%66.7)'sinin çay bahçesinde, 6(%33-3)'sının bahçe ya da çalılık yerlerde, en çok Mayıs-Eylül aylan arasında ve 10.00-12.00 ile 15.00-18.00 saatleri arasında meydana geldiği tespit edilmiştir. Sokma yeri açısından bakıldığında, 14 (%77.8) olgunun tek ayağında, l (%5.6) olgunun iki ayağında ve 3 (%16.7) olgunun da tek elinde sokma olduğu görülmüştür, ilk l saat içerisinde bulantı, kusma, ishal, hipotan-siyon, bayılma hissi, panik ve ağrı gibi şikayetleri olan olgular ortalama 2.5 (0.5-18 saat) saatte bir sağlık kurumuna başvurmuş ve ilk müdahaleden ortalama 6.5 (1-20 saat) saat sonra kliniğimize gelmişlerdir, ilk müdahalelerinde tümüne steroid ve antihistaminik uygulanan olguların 7 (%38.9)'sine antivenom, 14 (%77.8)'üne antibiyotik tedavisi kliniğimizde ilave edilmiştir. Tetanoz immünizasyonu 15 (%833) olguya uygulanmıştır. Selülit veya tromboflebit gelişen 14 (%77.8) olguya antibiyotik tedavisi başlanmıştır. Olgulann 10 (%55.6)'unda pitozis görülmüştür. Olgular ortalama 4 (2-10 gün) günde iyileşme sürecine girmiş ve hepsinde şifa gözlenmiştir. Yorum: Yılan sokmalan ile ilgili yurdumuzda ilk olan bu çalışmayla ülkemizde bulunan yılan çeşitleri, yılan sokması olgularına tedavi yaklaşımları tartışılmış ve mevcut olan antivenomlann yetersiz olabileceğine dikkat çekilmiştir.
