The value of the nested polymerase chain reaction in the diagnosis of ocular toxoplasmosis

Abstract

Out of the patients who presented at the ophthalmology departments of the medical faculties, of the Ege and Dokuz Eylül Universities, 35 patients were selected randomly. An expert ophthalmologist obtained aqueous humor samples from one eye only of 35 patients with cataracts before surgery and from both eyes of a patient who was given the presumptive diagnosis of acute ocular toxoplasmosis. These samples were examined at the Public Health Laboratory Service (PHLS) Toxoplasma Reference Unit in Swansea-Wales. The nested polymerase chain reaction (PCR) was used to detect Toxoplasma DNA and the ELISA for detection of anti-Toxoplasma IgG antibodies. The serum samples of these 36 patients were examined for the presence of Toxoplasma specific IgG and IgM antibodies using the dye test, IFAT IgG, ELISA IgG, ELISA IgM, immune capture ELISA (Toxonostika®), and ISAGA IgM (Bio-Merieux®). No anti-Toxoplasma IgM antibodies were detected in any of the samples. Of the 36 sera, 25 (69.4%) were positive for anti-Toxoplasma IgG antibodies, and this was considered to be evidence of chronic latent toxoplasmosis. One patient with a cataract showed a retinal lesion resembling a congenital toxoplasmic scar during surgery, but he was found to be serologically negative, and the presumptive diagnosis was excluded. The patient who showed signs of acute ocular toxoplasmosis in his right eye, and a congenital toxoplasmic scar on the left, was seronegative for IgM but seropositive for IgG and was considered to have chronic latent toxoplasmosis. Humor aqueous samples obtained from both eyes of this patient were found to be negative for antibodies by ELISA IgG, but positive for Toxoplasma DNA by nested PCR. Resolution of signs and symptoms were observed after specific anti-toxoplasmic therapy. All 35 samples of humor aqueous were negative both by nested PCR and ELISA, and no signs of toxoplasmosis were noted. As a result, nested PCR seems to be beneficial in the diagnosis of atypical cases of toxoplasmic chorioretinitis, but more cases should be examined.

Ege ve Dokuz Eylül Üniversitesi Tıp Fakültelerinin Göz Hastalıkları Anabilim Dallarına başvuran hastalardan rasgele seçilen 35 kataraktlı olgudan operasyon sırasında alınan birer ve akut oküler toxoplasmosis ön tanılı bir hastanın her iki gözünden lokal anestezi sonrası yapılan ön kamara parasentezi ile elde edilen iki, toplam 37 akköz hümör örneği Swansea-Galler, İngiltere'deki Public Health Laboratory Service (PHLS) Toxoplasma Referans Ünitesinde Nested PZR (Polimeraz Zincir Reaksiyonu) uygulanarak Toxoplasma DNA'sı, ELISA IgG testi ile de anti-Toxoplasma IgG antikorları açısından araştırılmıştır. Aynı 36 hastanın serum örnekleri Dye test (DT), IFAT IgG, ELISA IgG, ELISA IgM, immune capture ELISA (Toxonostika®), ISAGA IgM (Bio-Merieux®) testleri ile anti-Toxoplasma antikorları açısından değerlendirilmiştir. Hiçbir serum örneğinde anti-Toxoplasma IgM antikorlarının varlığı saptanamamış, 36 hastanın 25'inde (%69.4) anti-Toxoplasma IgG antikorları görülmüştür. Elde edilen veriler ışığında tüm seropozitif olgular kronik inaktif toxoplasmosis olarak yorumlanmıştır. Katarakt operasyonu sırasında bir olgunun retinasında konjenital Toxoplasma skarı ile uyumlu olduğu düşünülen lezyon görülmüş, serolojik testlerde anti-Toxoplasma antikorlarına rastlanılmayınca konjenital Toxoplasma skarı ön tanısından uzaklaşılmıştır. Araştırmaya alınan 36 hastadan sadece birinde, oftalmoskopik muayene sonucu sağ gözde akut toxoplasmosis, sol gözde ise konjenital toxoplasmosis skan olduğu düşünülmüştür. Bu hastanın serolojik olarak araştırılmasında anti-Toxoplasma IgM antikorları negatif, anti-Toxoplasma IgG antikorlan ise pozitif olarak saptanmış ve olgu kronik inaktif toxoplasmosis olarak değerlendirilmiştir. Her iki gözden alınan akköz hümör örneğinde ELISA IgG testi ile anti-Toxoplasma IgG antikorlarının varlığı saptanamamıştır. Aynı örneklerin nested PZR ile incelenmesinde ise her iki örnekte Toxoplasma DNA'sının varlığı gösterilmiş, özgül anti-Toxoplasma tedavisi sonrasında klinik ve semptomatik iyileşme görülmüştür. Katarakt operasyonu sırasında 35 hastadan alınan akköz hümör örneklerinin hiçbirinde nested PZR ile Toxoplasma DNA'sına ve serolojik yöntemlerle intraoküler anti-Toxoplasma antikorlarına rastlanılmamıştır. Sonuçta atipik lezyonları olan hastalarda Toxoplasmik retinokoroidit tanısının PZR ile konabileceği düşünülmüş, daha fazla olgu ile yapılacak çalışmaların daha yararlı olacağı kanaatine varılmıştır.
