Investigation of the etiology of nocturnal enuresis and evaluation of its relationship to Enterobius vermicularis

Abstract
Noctural enuresis (urinary incontinence during sleep) is an important and widespread condition affecting children and their families. Even though such conditions as physical immaturity, severe obstructions of the urinary tract and psychiatric disturbances have been considered in its etiology, it is still not completely clear. Recent studies have reported that Enterobius vermicularis (EV) may play a role in its etiology. The aim of this study was to investigate the etiology of nocturnal enuresis in children and to determine whether there is a relationship between enterobiasis and urinary infections with nocturnal enuresis. The EV incidence in 61 children who had presented at the Urology Clinic of the Medical Faculty of the Mersin University was determined using the cellophane tape method. They were examined 3 times every other day and EV was found in 19 (31.00%). Urinary infection was found in 8.19% of those who had had urine culturing done. Nocturnal enuresis had been present in 90.2% from birth but had occurred later in 9,8%. There was a history of nocturnal enuresis in 18.0% of the mothers, in 14.7% of the fathers and in both parents in 4.9%. Of the control group consisting of 60 children in the same age group, EV was found in only 6 (10%). Thus EV should be considered when the cause of nocturnal enuresis is being investigated,

Geceleri, uykuda idrar kaçırma anlamına gelen Enürezis nokturna çocukların ve ailelerin yaşamlarını etkileyen önemli ve yaygın bir bozukluktur. Enürezis etiyolojisinde fiziksel immaturite, ciddi üriner sistem obstrüksiyonu ve psikiyatrik bozukluk sorumlu tutulmasına rağmen etiyoloji tam olarak açığa kavuşmamıştır. Son yıllarda yapılan çalışmalarla enürezis nokturna etiyolojisinde Enterobius vermicularis'in de (E. vermicularis) etkisinin olabileceği bildirilmektedir. Bu çalışma, Bölgemizdeki, çocuklarda Enürezis nokturna etiyolojisini araştırmak ve Enürezis nokturna ile enterobiosis ve üriner sistem enfeksiyonu arasındaki ilişkiyi saptamak amacıyla planlanmıştır. Mersin Üniversitesi Tıp Fakültesi üroloji kliniğince takip edilen Enürezis nokturna yakınması görülen 6-12 yaşları arasındaki 61 çocukta E. vermicularis insidansı selofan bant yöntemiyle, üç kez birer gün ara ile araştırılmış ve 19 olguda (%31), E, vermicularis saptanmıştır. Olgulardan yapılan idrar kültürlerinde %8, 19'ında üriner infeksiyon tespit edilmiştir. Olguların %90, 2' inde doğuştan, %9, 8 'inde sonradan Enürezis nokturna görüldüğü, %18'inde annede, %14, 7'sinde babada, %4, 91'inde ise hem anne hem babada Enürezis nokturna hikayesi mevcut olduğu, %4, 9'unda spina bifida olduğu belirlenmiştir. Kontrol grubu olarak alınan aynı yaş grubundan 60 çocuğun, sadece 6'sında (%10) E. vermicularis görülmüştür. Enürezis nokturna nedenleri arasında, E. vermicularis'in de dikkate alınması gerekmektedir.
