Localized hypertrophic sciatic neuropathy: Case report and long term follow-up

Abstract
Localized hipertrophic mononeuropathy is a focal neuropathy characterized by concentric proliferation of Schwann cells around the axon. Such lesions are very rare in the absence of generalized hypertrophic neuropathy. Very little is known regarding the etiology and the natural course of this disorder. Our case had progressive focal neurological deficits that localized to tibial portion of sciatic nerve. Serious axonal loss and denervation findings were found in electrophysiologic evaluation suggesting a focal lesion of the tibial nerve. Magnetic resonance imaging from lower lumbar spine to the popliteal region demonstrated marked enlarged sciatic nerve showing abnormal hyperintense signal along thigh segment. The patient was diagnosed as the localized hypertrophic sciatic neuropathy. Pain was decreased by using medical and physiotherapy modalities, but muscle weakness progressed during the follow-up period.
Özet
Lokalize hipertrofik nöropati akson etrafındaki Schwann hücrelerinin proliferasyonu ile karakterize fokal bir nöropatidir. Bu tip lezyonlar generalize bir polinöropatinin yokluğunda çok nadir olarak görülmektedir. Hastalığın etyoloji ve doğal seyrine ait çok az şey bilinmektedir. Bizim olgumuzun siyatik sinirin tibial dalına lokalize, ilerleyici fokal motor defisiti vardı. Elektrofizyolojik değerlendirmesinde tibial sinirde fokal lezyon düşündüren ağır akson kaybı ve denervasyon bulguları tespit edildi. Alt lomber omurgadan popliteal bölgeye kadar yapılan manyetik rezonans incelemesi siyatik sinirin uyluk segmenti boyunca anormal hiperintens sinyal göstererek belirgin şekilde kalınlaştığını gösterdi. Hastaya lokalize hipertrofik siyatik nöropati teşhisi konuldu. Ağrı, medikal tedavi ve fizyoterapi modaliteleri kullanılarak azaltıldı, fakat kas güçsüzlüğü izlem süresi boyunca ilerledi.
