Intrathecal baclofen therapy in patients with traumatic spinal cord injury

Abstract
Objective: To evaluate the results of intrathehal baclofen (ITB) therapy in 15 patients with traumatic spinal cord injury (SCI). Materials and Methods: Medical records of 15 patients with SCI receiving ITB were evaluated retrospectively. Degree of spasticity before and after ITB, time interval between SCI and ITB, follow-up period after ITB, first effective dose of baclofen, current dose of baclofen and the complications after ITB were obtained. Results: Eight (53%) patients had tetraplegia and seven (47%) patients had paraplegia. The mean follow-up period after ITB was 57.43±36.89 months. According to the modified Ashworth scale (MAS) mean spasticities were 3.47±0.51 before ITB and 1.50±0.51 after ITB. Thedifference between the mean degree of spasticity before and after ITB was significant (p=0.001). The mean first effective dose of baclofen was 174.33±92.25 μg/d. The mean current dose of baclofen was240.42±125.19 μg/d. The differences in the mean first baclofen dosesand current baclofen doses were significant between complete andincomplete patients (p=0.012; p=0.018, respectively). Complications developed in nine ( 60%) patients. Conclusion: ITB is an effective treatment in traumatic SCI patients with severe spasticity, however, some medical and pump system complications might develop.

Amaç: İntratekal baklofen (İTB) tedavisi uygulanmış olan travmatik spinalkord yaralanmalı (SKY) 15 hastaya ait verileri değerlendirmektir. Gereç ve Yöntem: İTB tedavisi uygulanmış olan 15 travmatik SKY’li hasta retrospektif olarak tarandı. İTB uygulaması öncesi ve sonrası spastisite derecesi, SKY zamanı ile İTB uygulaması arasında geçen süre,İTB uygulaması sonrası takip süresi, İTB uygulaması sonrası spastisiteninazalmasını sağlayan etkili ilk baklofen dozu, en son uygulanmakta olan baklofen dozu ve İTB uygulaması sonrası takip süresince gelişen İTBkomplikasyonları belirlendi. Bulgular: Sekiz hasta tetrapleji (%53), yedi hasta parapleji (%47) idi. İTB sonrası takip süresi ortalama 57,43±36,89 aydı. Spastisite düzeyimodifiye Ashworth skalasına (MAS) göre İTB öncesi ortalama 3,47±0,51,İTB sonrası ortalama 1,50±0,51 olup, aralarındaki fark istatistiksel olarakanlamlıydı (p=0,001). Etkili ilk baklofen dozu ortalama 174,33±92,25μg/gün, son baklofen dozu ortalama 240,42±125,19 μg/gün idi. Etkili ilkve son baklofen dozu açısından komplet ve inkomplet hastalar arasındakifark istatistiksel olarak anlamlıydı (sırasıyla p=0,012; p=0,018). 9 hastada(%60) takip süresince komplikasyon izlendi. Sonuç: İTB tedavisi travmatik SKY’li hastalarda şiddetli ve inatçı spastisitetedavisinde etkili bir tedavi yöntemi olmakla beraber; bazı tıbbi vepompa sistemine ait komplikasyonlar gelişebilmektedir.
