Carcinoma of ampulla of vater arising from villous adenoma of intraduodenal bile duct

Abstract

One of the most unusual sites of isolated villous adenomas is the ampulla of Vater, Tumors of the ampulla of Vater are diagnosed earlier and have a good prognosis than periampullary region neoplasms because the relatively small neoplasm can easly obstruct the orifice of the papilla. A 69-year-old male patient presented with ichter for two months. Ultrasonographic examination revealed hydrops of the gallbladder. The pancreatic head was obliterated with sphincteroplasty and duct reimplantation was performed. Stage II carcinoma which is located in the bile duct openning to the ampulla of Vater was found. A rare case of ampulla of Vater was discussed with clinical, histopathological and prognostic features.

Özet

Ampuller bölgede yer alan vilöz tümörler, ampuller açıklığı küçük boyutta iken tıkamaları nedeniyle daha erken tanı alırlar ve periampuler bölge tümörlerinden daha iyi prognoz gösterir. 69 yaşında erkek hasta 3 aydan beri sarılığının olması nedeniyle başvurdu. Ultrasonografide safra kesesi hidropikti. Koledok distali ile pankreas loju oblitere olmuştu. Tümör belirleyicilerinden CA 125 ve GIM yüksek olarak bulundu. Lokal eksizyon sfinkteroplasti ve duktusa reimplantasyon uygulandı,. Ampulla Vater'e açılan safra kanalındaki tubulovillöz adenomdan gelişen evre II karsinom olgusu az görülen lokalizasyonu nedeniyle klinik, histopatolojik, prognozu belirleyen özellikleriyle tartışıldı.
