Abstract

Obstructing the orifice or the ductal lumen, carcinoma of the ampulla of Vater can be diagnosed early and unlike other periampuller neoplasms and colorectal carcinomas have a good prognosis. These neo-plasms are rare and make up about % 10,2-% 36 of all operable pancreaticoduoenal neoplasms. Adenomas and microadenomatous structures have been found in the vicinity of the carcinomas in 42-91 per cent of all cases. 64-year-old male patient presented with carci-noma of duodenum and ampulla of Vater which is localized on a microadenomatous proliferation . This case is presented here because of rarity and different features related with its localization.

Özet

Ampulla Vater karsinomları erken evrede ampulla lümenini tıkayarak semptom vermeleri nedeniyle periampuller bölge tümörlerinden ve kolorektal karsinomlardan daha iyi prognoz gösterir. Çok az görülen tümörler olup, öpere edilebilen pankreatoduodenal tümörlerin % 10,2-% 36 sini oluşturur. Mikroadenom ve intramukozal adenomlarm %42-91 oranında bu invaziv lezyonlara eşlik ettiği gösterilmiştir. 64 yaşında erkek hastada tanımlanan ampulla Vater'deki mikroadenomatöz proliferasyon zemininde ve duodenumda iki odak halinde gelişmiş evre II karsinom, yerleşim yerine bağlı olan değişik özellikleri ve az görülmesi nedeniyle kaynak bilgileri eşliğinde sunuldu.
