Evaluation of home health service care children with tracheotomy and mechanical ventilator

Abstract
Objective: The aim of this study was to evaluate the clinical character-istics of children with tracheotomy, dependent to mechanical ventila-tor, taking home care service, their families’ socioeconomic status and attitudes about disease process in Samsun.Methods: Atotal of 32 pediatric patients with tracheotomy dependent to mechanical ventilation who were enrolled in the Home Health Ser-vices Unit of Samsun Training and Research Hospital between June 2015 and June 2019 were included in the study. After the families of the pediatric patients were informed, verbal and written consent was obtained. The questionnaire consisting of 19 questions was filled by asking the parents during the home visit by the researcher. Demo-graphic and medical data of children, socio-demographic and eco-nomic characteristics of the families, their education about the disease process and their psychological status were evaluated by the question-naire. Statistical evaluation was made by giving percentage rates.Results: In our study group, there were 19male and 13female pa-tients aged between 1 and 16years. While 28 of the pediatric patients were followed up due to mechanical ventilation, 4 pediatric patients were followed up only by tracheotomy. During the 48-month follow-up period, 5 ventilator-dependent pediatric patients died and the trac-heotomy of 2 pediatric patients was closed. When the primary diseases of our pediatric patients were grouped, 16(50%) were neurological, 6(18.7%) were respiratory diseases, 5(15.6%) were muscle diseases and 5(15.6%) were metabolic diseases. Thirty-one (96.8%) families with tracheotomy and/or ventilator-dependent pediatric patients who continued their care at home reported that their work and social lives were affected; while one family reported as not affected. It was found that the mothers took care of all the patients and 31(96.8%) of the mothers stated that their education related to the disease process and the maintenance and use of medical devices were sufficient. The fat-hers stated that they could not go to work and even they were at work they had to come home in unexpected situations of children. Mothers, on the other hand, stated that they could not leave their child alone in any way, because they could not get out of the house, their neighbors did shopping for them. The results showed that all of the pediatric patients were cared by mothers and 31 of the mothers (96.8%) did not consider themselves sufficient, despite being educated on the disease process, the care and use of medical devices, before their discharge.Conclusion: We consider that there is a need for comprehensive studies to reduce the repeated hospitalization of children with trac-heotomy and mechanical ventilators, to provide in-service training to the relevant healthcare workers within the home healthcare services; and to provide economic, social and psychological support to the fa-milies providing care.

Amaç: Bu çalışmada Samsun ilinde evde sağlık hizmeti ve-rilen trakeotomili ve mekanik ventilatöre bağımlı çocuk has-taların klinik özellikleri, bakım veren ailelerinin sosyoekono-mik durumları ve hastalık süreci ile ilgili tutumlarının değer-lendirilmesi amaçlanmıştır.Yöntem: Haziran 2015-Haziran 2019 tarihleri arasında Sam-sun Eğitim ve Araştırma Hastanesi Evde Sağlık Hizmetleri birimine kayıtlı, evlerinde takip edilen trakeotomili ve mekanik ventilatöre bağımlı toplam 32 çocuk hasta retrospektif olarak değerlendirilmiştir. Çocuk hastaların ailelerine bilgi verilip sözlü ve yazılı onamları alındıktan sonra 19 sorudan oluşan anket formu araştırıcı tarafından ev ziyareti esnasında ebeveynlere sorularak dolduruldu. Anket soruları ile çocukların demografik ve tıbbi ve-rileri, ailelerin sosyo-demografik ve ekonomik özellikleri, hastalık süreciyle ilgili eğitimleri ve psikolojik durumları değerlendirildi. İstatistiksel değerlendirme yüzdelik oranlar verilerek yapıldı.Bulgular: Çalışma grubumuzda yaşları 1 ile 16 arasında değişen, 19’u erkek (%59,3), 13’ü kız (%40,6) hasta bulunmaktadır. Çocuk hastaların 28’i mekanik ventilatöre bağlı takip edilirken, 4 çocuk hasta sadece trakeotomi ile takip edilmektedir. Kırk sekiz aylık takip sürecinde 5 ventilatöre bağımlı çocuk hasta vefat etmiş ve 2 çocuk hastanın trakeotomisi kapatılmıştır. Takip edilen çocuk hastalarımızın primer hastalıkları gruplandırıldığında 16’sının (%50) nörolojik, 6’sının (%18,7) solunum yolları hastalığı, 5’inin (%15,6) kas hastalığı ve 5’inin de (%15,6) metabolik hastalıkları olduğu tespit edildi. Trakeotomili ve ventilatöre bağımlı çocuk hastaların bakımına evlerinde devam eden ailelerin 31’i (%96,8) iş ve sosyal hayatlarının etkilendiğini, biri ise etkilenmediğini bildirdi. Ebeveynlerden baba, çocuğun beklenmedik durumlarında işe gidemediğini, işte ise de eve gelmek zorunda kaldığını bildirdi. Anneler ise çocuğunu hiçbir şekilde yalnız bırakamadığını, ev dışına çıkamadığını, alışverişlerini bile çoğunlukla komşularının yaptığını belirtti. Çocuk hastaların tamamına annelerinin baktığı, taburcu olmadan önce annelerin 31’inin (%96,8) hastalık süreci ve tıbbi cihazların bakımı ve kullanımı ile ilgili eğitim almalarına rağmen kendilerini yeterli görmediklerini bildirdiler.Sonuç: Trakeotomili ve mekanik ventilatöre bağımlı çocukların hastaneye mükerrer yatışlarını azaltmak, evde sağlık hizmetleri bünyesindeki ilgili sağlık çalışanlarına konu ile ilgili hizmet içi eğitimler vermek ve bakım veren ailelere ekonomik, sosyal ve psikolojik destek sağlamak için geniş kapsamlı çalışmalar yapılmasına gereksinim olduğu kanaatindeyiz.
