Abstract

Objectives: Knowledge of factors that effect the development of complications while inserting a central venous catheter can lead to interventions which can lead to decreased rates of complications. In this study we evaluate the factors that effect the development of complications during central venous catheter insertion. Materials and Methods: 325 patients (179 males (%55.1), 146 females (%44.9)) who had central venous catheters inserted in Dide University hospital emergency department between June 2005 and May 2006 were enrolled in this study. The effect of age, gender, reason for central venous catheter insertion, diameter of the catheter, the initial insertion site, the number of attempts for inserting the catheter, the time of day when the catheterwas inserted and the level of training of the physician on development of complications was studies. Chi-square test was used for categorical variables and the t test for continuous variables in statistical analysis. Results: Complications developed in 12% (n=39) patients. Of patients with complications, 56.4% (n=22) were male and 43.6% (n=17) female. No significant statistical relationship was detected between complication development and age, gender, reason of central venous catheter insertion, diameter of the catheter, the insertionsite, the time of day when the catheter was inserted, or the level of training of the physician (p>0.05). Patients in whom the initial insertion site was infraclavicular (42.6%) had a statistically significant higher rate of complications (p=0.036). Giriş: Santral venöz kateter (SVK) takılan hastalarda komplikasyon gelişimi üzerinde etkili faktörleri önceden bilmek ve bu bilgi doğrultusunda girişimde bulunmak komplikasyonları en aza indirmek için önemlidir. Çalışmamızda SVK takılan hastalarda komplikasyon gelişimi üzerinde etkili faktörleri araştırmayı amaçladık. Gereç ve Yöntem: Haziran 2005 ile Mayıs 2006 tarihleri arasında Dicle Üniversitesi Hastanesi Acil Servisinde SVK takılan 325 hasta (179 erkek (%55.1), 146 kadın (%44.9)) çalışmaya alındı. Hastanın yaşı, cinsiyeti, SVK takılma nedeni, kullanılan kateter çapı, kateter takmak için kullanılan lokalizasyon, ilk tercih edilen lokalizasyon, yapılan girişim sayısı, girişimin yapıldığı saat, girişimi yapan doktorun kıdeminin komplikasyon gelişimi üzerine olan etkileri araştırıldı. İstatistiksel değerlendirmede, kategorik değişkenler için Ki-kare testi (%2), sürekli değişkenler için t testi kullanıldı. Bulgular: Santral venöz kateter takılan 325 hastanın 39'unda (%12) komplikasyon gelişti. Komplikasyon gelişimi ile hastanın yaşı, cinsiyeti, SVK takılma nedeni, kullanılan kateter çapı, kateter takmak için kullanılan lokalizasyon, girişimin yapıldığı saat ve girişimi yapan doktorun kıdemi arasında istatistiksel olarak anlamlı bir ilişki saptanmadı (p>0.05). İlk infraklavikular yol tercih edilen hastaların (%42.6) komplikasyon gelişim oranı (p=0.036) ve kateter takma girişimi üç ve daha fazla olan hastaların (%26.5) komplikasyon gelişim oranı istatistiksel olarak anlamlıydı (p=0.000). Sonuç: Santral venöz kateter takarken ilk infraklavikular girişimin tercih edilmesi ve üç ya da daha fazla girişimde bulunulması komplikasyon gelişimi üzerine etkili faktörlerdir
