Amebiasis and the role of virulance factors in Entamoeba histolytica pathogenesis

Abstract
Amebiasis is a parasitic disease of worldwide which has public health importance,particularly in developing countries havingpoor sanitary conditions. Clinical amebiasisresults from the spread of trophozoites into the colon wall and beyond, and also the parasitemay invade other organs such as liver, brain and causes abscess. Current development onE. histolytica pathogenesis is based on the publication of E.histolytica genome and usingmolecular techniques such as antisense inhibition of target gene expression and irreversiblegene silencing. After characterization of E. dispar as a distinct species that is morphologicallyidentical but apathogen in 1993, difference between two microorganisms in genomic levelhas revealed much information about virulence factors. In the light of recent studies, fourvirulence factors are defned; adhesion molecules, amoebapores, cysteine proteinasesand complex lipophosphopeptidoglycans. The roles of these factors during invasion arethe main concept of this review. Furthermore, interactions between parasite and hostcell are summarized; this is the key point of understanding the role of infammation andprogrammed cell death in the pathogenesis of amebiasis. Although we have learned much about pathogenesis in recent years, there are still many unanswered questions. Most important ones of those questions areabout encystation of parasite, infammation process, apoptosis and necrotic cell death of host cells. In addition, how theparasite causes different clinical signs in different organs is another unexplained part in pathogenesis. Consequently, newstudies and realistic animal models are needed to clarify the pathogenesis of E. histolytica.

Amebiyaz, özellikle sanitasyonun eksik olduğu gelişmekte olan ülkelerde sık görülenönemli bir halk sağlığı problemidir. Hastalığın kliniği parazitin bağırsak yüzeyine ve diğertabakalarına yayılmasıyla ortaya çıkar. Bununla birlikte parazit karaciğer, beyin gibi diğerorganlara da yerleşerek apse oluşturabilir. Parazit genomunun dizilenmesi, antisens geninhibisyonu ve gen susturulması gibi moleküler yöntemlerin kullanılması sonucu parazitinpatogenezinin anlaşılmasında büyük gelişmeler sağlanmıştır. E. dispar’ ın 1993’de morfolojikolarak benzer fakat apatojen bir tür olarak tanımlanmasından sonra, aralarındaki genomikdüzeydeki farklar virülans faktörlerinin açıklanmasında önemli bir yer tutmaktadır. Sonçalışmalar ışığında E.histolytica’da dört grup virulans faktörü tanımlanmıştır. Bunlar;adezyon molekülleri, amebaporlar, sistein proteinazlar ve lipofosfopeptidoglikan yüzeykompleksleridir. Bu faktörlerin lezyonun gelişimi sırasındaki rolleri bu derlemenin temelkonusunu oluşturmaktadır. Ayrıca, parazit ve konak hücre etkileşimleri burada özetlenmiştir;bu etkileşimin açıklanması infamasyon ve programlı hücre ölümünün gerçekleşmesindeki enönemli noktadır. Son yıllarda parazitin patogenezi hakkında daha fazla bilgiedinilmesinerağmen cevap bekleyen birçok soru bulunmaktadır. Bunların başında parazitin enkistasyonu,infamasyon gelişimi, apopitotik ve nekrotik hücre ölümü gelmektedir. Ayrıca, parazitinyerleşim yerine göre farklı klinik belirtiler vermesi, açıklanması gereken bir diğer konudur.Bu nedenle, E. histolytica patogenezinin daha iyi anlaşılması için yeni çalışmalara vegerçekçi hayvan modellerine ihtiyaç vardır.
