Evaluation of anti-Toxoplasma IgM and IgG seropositivity among women in reproductive period, who admitted to Süleyman Demirel University Hospital

Abstract

Objective: Toxoplasma gondii causes generally an asymptomatic infection. If it develops during pregnancy, it can cause some fetal disorders. In the early stage of pregnancy, the risk of spreading of infection to the fetus is low while the possibility of appearance of serious symptoms is high. In the late stages of pregnancy, the risk of speading of infection is high while the appeared symptoms are mild even asymptomatic. This descriptive study was planned aiming to determine anti-T. gondii IgG and IgM seropositivity in the women at the ages between 15-49 who applied to the Süleyman Demirel University Medical Faculty Hospital.Methods: The anti-Toxoplasma IgM, antiToxoplasma IgG and anti-Toxoplasma avidite test results performed on the serum samples of the women in the reproductive period which were send from several clinics to our hospital medical microbiology laboratory between the dates 1 January - 31 December 2013, were evaluated retrospectively. Anti-Toxoplasma IgM and anti-Toxoplasma IgG tests (Vitros, Johnson & Johnson, ABD) were performed with chemiluminescence method while anti-Toxoplasma IgG avidite test were performed with enzyme-linked fluorescent assay (ELFA) method (VIDAS, bio-Merieux France) and the evaluations were done in the direction of the company recommendations. Results: In our study, in the women having average age of 29.78 ± 5.95 the rate of positivity of anti-Toxoplasma IgM and rate of anti-Toxoplasma IgG seropositivity were determined as 5.2% (45/862) and 24.4% (194/794) respectively. In the anti-Toxoplasma IgG avidity test demanded women in the rate of 84.6% (55/65) high avidity, in the rate of 12.3% (8/65)low avidity and in the rate of 3.1% (2/65) avidity value in the limit were determined. Conclusion: It was observed that among the women at the age between 15-49 who applied to our hospital, the group who is sensitive to the T. gondii infection is not so small. Since low avidity values were also determined in our study, it was reached to the conclusion that especially in the reproductive age group women congenital toxoplasmosis should be brought to mind.

Amaç: Toxoplasma gondii enfeksiyonu, insanlarda genellikle klinik belirti vermemekle birlikte gebelikte geçirildiğinde fetüs üzerinde olumsuz etkiler oluşturabilmektedir. Gebeliğin erken döneminde enfeksiyonun fetüse bulaşma riski düşük, ciddi semptomların ortaya çıkma olasılığı ise yüksektir. Gebeliğin geç dönemlerinde enfeksiyonun fetüse bulaşma riski yüksek, ortaya çıkan bulgular ise hafif hatta asemptomatik olabilmektedir. Bu tanımlayıcı çalışma, Süleyman Demirel Üniversitesi Tıp Fakültesi Hastanesi'ne başvuran, 15-49 yaş arası kadınlarda anti-T. gondii IgG ve IgM seropozitifliğinin belirlenmesiamacıyla planlanmıştır. Yöntemler: Hastanemiz Tıbbi Mikrobiyoloji Laboratuvarına, 1 Ocak-31 Aralık 2013 tarihleri arasında, çeşitli kliniklerden gönderilen, doğurganlık çağındaki kadınlara ait serum örneklerinde çalışılan anti-Toxoplasma IgM, anti-Toxoplasma IgG ve antiToxoplasma IgG avidite test sonuçları geriye dönük olarak değerlendirmeye alınmıştır. Anti-Toxoplasma IgM ve anti-Toxoplasma IgG testleri (Vıtros, Johnson & Johnson, ABD) kemilüminesans yöntemi ile, antiToxoplasma IgG avidite testi (VIDAS, bio-Merieux, Fransa) ise enzim bağlantılı floresan testi (ELFA) yöntemi ile çalışılmış ve firma önerileri doğrultusunda değerlendirilmiştir. Bulgular: Çalışmamızda, yaş ortalaması 29,78 ± 5,95 olan kadınlarda %5,2 (45/862) oranında antiToxoplasma IgM pozitifliği, %24,4 (194/794) oranında anti-Toxoplasma IgG seropozitifliği tespit edilmiştir. Anti-Toxoplasma IgG avidite testi istenen kadınlarda ise %84,6 (55/65) oranında yüksek avidite, %12,3 (8/65) oranında düşük avidite ve %3,1 (2/65) oranında sınırda avidite değeri tespit edilmiştir. Sonuç: Hastanemize başvuran 15-49 yaş grubu kadınlar arasında T. gondii enfeksiyonuna duyarlı grubun hiç de küçük olmadığı gözlenmiştir. Çalışmamızda düşük avidite değerleri de saptanmış olup özellikle doğurganlık yaş grubu kadınlarda konjenital toksoplazmozun akla getirilmesi gerektiği sonucuna varılmıştır.
