Prognostic value of serum CEA and CA 19-9 levels in patients with advanced colorectal cancer

Abstract
In this study, the prognostic value of the serum levels of CEA and CA19-9 in 55 patients with advanced colorectal cancer was investigated. Tumor markers were taken at the presentation. In univariate analysis, CEA, CA 19-9, poor differentiated tumor histology, and performance status were found to be of prognostic significance. In the Cox analysis, CEA, tumor histology, and performance status were retained as independently variables. CEA levels were correlated with a worse prognosis at the cut-off value of 5 ng/ml (p=0.01; medyan survival 14 months vs 8 months). This study shows that serum CEA level at the presentation is an independent prognostic factors in patients with advanced colorectal cancer. Initial CEA levels may reflect not only tumor burden, but also tumor biology.
Özet
Bu çalışmada ileri evre kolorektal kanserli 55 hastada serum CEA ve CA 19-9 düzeylerinin prognostik değeri araştırılmıştır. Tümör belirleyicileri için serum örnekleri başvuru anında alınmıştır. Tekdeğişkenli analizde, CEA, CA 19-9, kötü differansiye tümör histolojisi ve performans durumunun prognostik önemi olduğu görülmüştür. Başvuru anında cut-off değeri 5 ng/ml'in üzerinde serum CEA değerleri kötü prognozla ilişkili bulunmuştur (p=0.01; medyan sağkalım 14 aya karşı 8 ay) Cox çokdeğişkenli analizde CEA, tümör histolojisi ve performans durumu bağımsız değişkenler olarak önemini korumuştur. Bu çalışma başvuru anındaki serum CEA düzeyinin ileri evre kolorektal kanserli hastalarda bağımsız bir prognostik faktör olduğunu göstermektedir. Başlangıç CEA düzeyleri yalnızca tümör yükünü değil, aynı zamanda tümör biyolojisini yansıtmaktadır.
