Comparison of the two treatments for patients with metastatic prostate cancer: Leuprolide acetate and flutamide vs bilateral orchiectomy and flutamid

Abstract
Fifty three patients with metastatic prostate cancer who did not receive treatment before were seperated into two groups. Twenty-eight of them were given leuprolid and flutamid and 25 of them underwent bilateral orchiectomy and maximal androgen blockage with flutamide. Patients were evaluated seperately in terms of survival and progression time and adverse effects in both groups. In leuprolide and flutamide group, mean progression time was found 14.2 months and mean survival time was 26.5 months. These values were 17.5 months and 28.2 months accordingly in the group with orchiectomy and flutamide. Toxicity was feasible in both treatment groups. Patient's age, social status and cost-effectiveness must be considered when taking a desicion about the kind of the treatment for patients with metastatic prostate cancer.
Özet
Metastatik prostat kanseri tanısı konulan ve daha önce herhangi bir tedavi uygulanmamış 28 hastaya leuprolid ve flutamid, 25 hastaya ise bilateral orşiektomi ve flutamid ile maksimal androjen blokajı tedavisi uygulandı. Hastalar sağkalım , progresyon süresi, yan etkiler açısından her iki grup içinde ayrı ayrı değerlendirildi. Leuprolid ve flutamid alan grupta ortalama progresyon süresi 14.2 ay, sağkalım süresi ortalama 26.5 ay olarak bulunurken bu değerler orşiektomi ve flutamid alan grupta sırasıyla 17.5 ay ve 28.2 ay olarak tesbit edildi. İstatistiksel açıdan değerlendirildiğinde hem progresyon hem de sağkalım süreleri açısından anlamlı bir fark bulunamadı. Yan etkiler her iki grupta da kabul edilebilir düzeydeydi. Maksimal androjen blokajı uygulanması düşünülen metastatik prostat kanserli hastalarda tedavi şeklinin hastanın yaşı , sosyal konumu , tedavinin maliyeti ve intermitan androjen blokajı uygulanıp uygulanmayacağı gibi değişik parametreler gözününe alınarak tercih edilmesi gerektiğine inanıyoruz.
