Auotologous blood transfusion in elective surgery

Abstract
Transfusion of blood from one person to another is mostly a problem of immunity and infection. The aim of the pre-sent study was to examine cost-effectiveness and transfusion reaction differences between the autologous and homologous blood transfusion, in patients who were scheduled for an elective surgical procedure. Methods: The study included 37 patients who underwent an elective general surgical procedure at Social Security Foundation Ankara Education Hospital during 1995-1996. The patients were divided demographically into two similar groups who received autologous (n-17) or homologous (n-20) blood transfusions and compared the results for transfusion reactions and cost-effectiveness. Results: The analysis of the results showed autologous blood transfusions, prepared from the same person a week before the elective surgical procedure, caused decreased transfusion reactions and lower costs compared to the homologue transfusion group. Conclusion: This study and previous observational studies support the hypothesis that perioperative homologous blood transfusion are associated with increased transfusions reactions and increased costs when compared with the autologous blood transfusions. Preoperative autologous donation of blood is a safe and cost-effective procedure for patients who undergo an elective surgical procedure.

Elektif şartlarda yapılan kan transfüzyonları, potansiyel bazı riskleri beraberinde taşımaktadır. Çalışmada, transfüzyon ihtiyacı, otolog veya homolog yollarla karşılanan iki ayrı gurubun, transfüzyon reaksiyonları ve her iki yöntemin maliyetleri karşılaştırıldı. Hastalar ve Yöntem: Elektif şartlarda operasyon için hazırlanan ve transfüzyon ihtiyacı olabileceği düşünülen 37 olgu, otolog (gurup-1, n=17) ve homolog (gurup-2, n=20) olmak üzere iki ayrı gruba ayrıldılar. Birinci guruptaki 17 olgudan, operasyon öncesi dönemde, ondördünden ikişer ünite (1000 cc) ve üçünden birer ünite (500 cc) olmak üzere, hemodülisyon yöntemi ile, toplam 31 ünite otolog kan depolanması yapıldı. Olgulara, hemodülisyon için, alınan her ünite kan miktarı için, 500 cc Isolyte ve 500 cc Ringer Laktat (Baxter-Eczacibasi") solüsyonları verildi. İntraoperatif dönemde transfüzyon ihtiyacı bulunması durumunda; birinci guruba otolog ikinci guruba ise homolog kan transtuzyonu uygulandı. Daha sonra guruplar; gösterdikleri transfüzyon reaksiyonları ve kan transfüzyon maliyeti açısından karşılaştırıldılar. Sonuçların analizinde; otolog kan transfüzyonu uygulanan olgularda, homolog kan transfüzyonu uygulanan olgulara göre daha az transfüzyon reasksiyonu gelişti ve otolog kan transfüzyonu uygulaması maliyetinin, homolog kan transfüzyonu maliyetine göre, daha az olduğu tespit edildi.
