The prognostic factors for local disease free survical and distant disease free survival in stage I-III breast cancer patients treated with adjuvant cyclophoshamide-adriamycin-5-fluorouracil (CAF) chemotherapy

Abstract
In this study, 248 breast cancer patients treated with adjuvant cyclophosphamide- adriamycin -5- fluorouracil (CAP) chemotherapy between 1987 and 2000 were evaluated for local disease free survival (LDFS), distant disease free survival (DDFS) and the prognostic factors affecting LDFS and DDFS. The mean age of the patients were 44.6 years (range: 21-75). The majority of the patients were premenopausal (71%). Estrogen receptor positivity was %48 in 151 patients. Progesterone receptor positivity was %47 in 130 patients. According to TNM classification 3.6%, 59.3% and 34.7% of the patients were at stage I, II and III, respectively. During the median follow-up time of 24.1 months (range: 4.8 - 156.2), 19 patients and 57 patients developed local recurrence and distant recurrence, respectively. In univariate analyses, statistically significant predictors for local disease free survival age, nodal status, number of focus of the tumor and receptor status; and for distant disease free survival age, nodal status, maximum size of metastatic lymph nodes, number of focus of the tumor and stage. In multivariate analyses, number of metastatic lymph nodes was the most significant independent prognostic factor for local disease free survival and distant disease free survival. While tumor multicentricity was another independent factor for local disease free survival, age and tumor multicentricity were the other independent prognostic factors for distant disease free survival. We conclude that the number of metastatic axillary lymph nodes, especially when more than 3, is the most significant prognostic factor in patients treated with CAF.

Bu çalışmada, 1987-2000 yılları arasında adjuvan siklofosfamid-adriamisin-5-Florourasil (CAF) kemoterapisi verilen 248 meme kanserli hasta lokal ve uzak nükssüz sağkalım ve bunları etkileyen prognostik faktörler açısından incelenmiştir. Yaşları 21 ile 75 arasında değişen ve yaş ortalaması 44.6 olan hastaların büyük çoğunluğu (%71) premenapozaldi. Östrojen reseptörü, çalışılan 151 hastanın %48'inde; progesteron reseptörü, çalışılan 130 hastanın %47'sinde pozitifti. Hastaların %3.6'sında evre I, %59.3'ünde evre II, %34.7'sinde evre III hastalık mevcuttu. Ortanca 24.1 (4,8-156,2) aylık takip sonrası, hastaların 19 (%7.7)'unda lokal nüks ve 57 (%23)'sinde uzak nüks gelişti. Tek değişkenli analizlerde, lokal nükssüz sağkalım açısından hastanın yaşı, metastatik lenf nodu sayısı, tümörün odak sayısı ve reseptör durumu; uzak nükssüz sağkalım açısından ise hastanın yaşı, metastatik lenf nodu sayısı, en büyük metastatik lenf nodunun çapı ve tümörün odak sayısı anlamlı bulundu. Çok değişkenli analizlerde metastatik lenf nodu sayısının, lokal nükssüz sağkalım ve uzak nükssüz sağkalım açısından en önemli prognostik faktör olduğu görüldü. Hastanın yaşı uzak nükssüz sağkalımda, tümör odak sayısı ise hem lokal hem de uzak nükssüz sağkalımda etkili olan diğer bağımsız prognostik faktörlerdi. Çalışmamızın sonucu, metastatik lenf nodu sayısının değişmeyen en önemli prognostik faktör olduğunu, tutulan aksiller lenf nodu sayısının özellikle üçten fazla olmasının çok anlamlı olduğunu göstermektedir.
