Protein C resistance and activity in elderly patients with acute stroke

Abstract
Hypercoagulability’s place, which is especially inquired in routine in the young patient population, is controversial in the elderly group of the acute stroke patients. In this study, the activity of natural endogen antikoagulan Protein C and the active Protein C resistance’s relationship to the age, characteristics of the lesions, clinical course, prognosis and other risk factors were investigated in adult group of acute ischemic stroke patients. The activity of Protein C and APC resistance was studied in 47 patients (22 female,25 male) which were diagnosed as acute ischemic stroke in the first three days of their stroke. The age average of the patients included in the study was found 66±16. In 10 patients the APC resistance was found to be abnormal according to the normal laboratory values. The Protein C activity was found anormal in 9 patients. When the patients having DM and those who didn’t, were compared according to the haemostatic parameters and Protein C activity, the diabetic patients were found to have statistically meaningful prominent low levels than the non-diabetic group (p=0.038). The prognostical assesment which was made to depend on the first ten days survival showed that the Protein C activity was prominently higher in the patients who survived than the patient who had died (p=0.017). The abnormality in Protein C activity was found in high rates in the patients who died. When the Protein C activity and the other laboratory parameters were inquired, negative corelation was found between the Protein C activity and lasting blood glucose levels, CRP and fibrinogen As a conclusion, Protein C and the other endogen anticoagulans are important with the APC resistance in young stroke patients, but in adult group patient population they interact with inflammatory, metabolic poor prognostic factors. We found that, the decrease in Protein C activation was not correlated to the increase in APC resistance. Our results conclude that the methods that will increase Protein C activation could be important in stroke treatments and may change the prognosis.The objective of this study is to show the improvement in the ambulation level and daily living activities of 88 hemiplegic patients who admitted to our clinic. Patients who are taken to the rehabilitation program and ≥65 years old are included. Their daily living activities were evaluated at each admission and discharge by using Barthel Index (BI) and their ambulation level by Functional Ambulation Scale (FAS). Özellikle genç hasta populasyonu arasında rutin olarak araştırılan hiperkoagulabilitenin, ileri yaş akut strok hasta grubundaki yeri tartışmalıdır. Bu çalışmada, endogen antikoagulan olan Protein C aktivitesi (PCA) ile aktive protein C rezistansının (APCR) yaş, lezyon özellikleri, klinik, prognoz ve diğer risk faktörleri ile bağlantısı, ileri yaş grubundaki akut iskemik strok hastalarında araştırıldı. Akut iskemik strok tanısı ile izlenen 22 kadın ve 25 erkek, toplam 47 hastada PCA ile APCR strokun ilk 3 gününde çalışıldı. Çalışmaya alınan hastaların yaş ortalaması 66±16 olarak tespit edildi. APCR değerleri hastanın 10’ nunda normal laboratuvar değerlerinin dışında, PCA ise 9 hastada anormal düzeyde bulundu. Prognostik değerlendirme olarak, ilk 10 günde hayatta kalmalarına göre yapılan gruplandırmada ise, PCA, hayatta kalanlarda eksitus olanlara göre belirgin olarak yüksekti (p=0.017). Diabetes mellitusu olan ve olmayan hasta grupları hemostatik parametreler ve PCA için karşılaştırıldığında, diabeti olan grupta olmayan gruba göre istatiksel olarak belirgin anlamlılıkta düşüklük tespit edildi (p=0.038). PCA’nın açlık kan şekeri, CRP ve fibrinogen ile negatif korelasyon gösterdiği tespit edildi. PCA anormalliği eksitus olan hastalarda yüksek oranda bulundu (p=0.05). Sonuç olarak, protein C ve diğer endogen antikoagulanların genç strok hastalarında aktive protein C rezistansı ile beraber önem taşımasının yanısıra, ileri yaştaki hasta populasyonunda diğer inflamatuar, metabolik kötü prognostik risk faktörleri ile etkileştiği sonucuna varıldı. Verilerimiz Protein C aktivasyonunu arttıracak yöntemlerin, strok tedavisinde önemli olabileceği ve prognozu değiştirebileceğini düşündürmektedir.Bu çalışma kliniğimize yatırılarak rehabilitasyon programına alınan 65 yaş ve üzeri 88 hemiplejik hastanın günlük yaşam aktivitelerinde ve ambulasyon düzeyinde gelişmeyi göstermek amacıyla planlandı. Günlük yaşam aktiviteleri; Barthel Indeksi (BI) ile, ambulasyon düzeyleri; Fonksiyonel Ambulasyon Skalası (FAS) ile giriş ve çıkışta değerlendirildi. Her hastaya kognitif fonksiyonları değerlendirmek için MMSE (Mini Mental Durum Testi) uygulandı.
