Multiple testicular sperm extraction and sperm retrieval rate according to testicular pathology in nonobstructive azoospermic men

Abstract
Objective: To investigate the relationship between spermatozoa retrieval ratio with testicular sperm extraction and testicular pathology. Institution: Gazi University School of Medicine, Department of Urology and Pathology. Material and Methods: Fourty nonobstructive azoospermic men who underwent multiple testicular sperm extraction were evaluated. The mean age of the patients was 32.5 ± 4.3 years (24-42). The mean testicular volume of patients was 12.9±3.4 cc. Serum FSH, LH, free testosterone and prolactin levels were 10.8±8.3 IU/L, 4.8±2.7 IU/L, 11.3±3.7 ng/mL and 6.2±2.l ng/mL respectively. All patients underwent chromosomal analysis (karyotype and Y chromosome deletions) preoperatively. Patients with Y chromosome microdeletions and karyotype abnormalities were excluded from our study. Intracytoplasmic sperm injection (ICSI) was performed when multiple TESE producers were successful. Testicular specimens of each patient were also evaluated pathologically. Results: The TESE procedure was successful in 21 patients. We retrieved spermatozoa from 7 of 16 patients (44%) with sertoli cell only. 9 of 12 patients (75%) with hypospermatogenesis, 5 of 12 patients (41%) with maturation arrest. Conclusion: In our study, according to testicular histology pattern, different sperm retrieval rates were observed after considering the other prognostic factors. Therefore, testicular histology pattern seems to be one of the important prognostic factor in sperm retrieval prior to testicular sperm extraction procedure in nonobstructive azoospermia.

Amaç: Nonobstrüktif azoospermik erkeklerde testiküler histopatoloji tiplerine göre sperm elde etme oranlarını karşılaştırmaktır. Çalışmanın yapıldığı yer: Gazi Üniversitesi Tıp Fakültesi Üroloji ve Patoloji Anabilim Dalları Materyal ve tnetod: Bu retrospektif klinik çalışmada nonobstrüktif azoospermi tanısı konulan 32.5 ± 4.3 (24-42) yaş ortalaması olan 40 hasta multiple testiküler sperm ekstraksiyonıı öncesinde incelenmiştir. Preoperatif olarak testis volümleri (!2.9±3.4), hormonal inceleme (serum FSH I0.8±8.3 fU/L, LH 4.8±2.7 I Uf L. Serbest Testesteron 11.3+3.7 ng/mL, Prolaktin 6.2±2.1 ng/mL) ve genetik araştırma (Karyotipi, Y kromozom mikrodelesyonu) yapılmıştır. Y kromozom mikrodelesyonu ve karyotipik anomalisi olmayan hastalara çalışmaya alınmıştır. Multiple testiküler sperm ekstraksiyonu (TESE) ile sperm bulunan çiftlerde intrastoplazmik sperm enjeksiyonu (ICSI) uygulanmıştır. Ayrıca her hastadan alınan testis materyalleri patolojik olarak değerlendirilmiştir. Bulgular: Sertoli cell only sendroınıı olan 16 hastanın 7'sinde (%44), hiposperrnatogenezisi olan 12 hastanın 9'unda (%75), matürasyon arresti olan 12 hastanın 5'inde (%41 ) spermatozoa bulunmuştur. Sonuç: Çalışmamızda sperm elde etmede kullanılan diğer prognostik faktörler inceledikten sonra oluşturulan hasta grubunda, testiküler histoloji paternine göre sperm elde etme oranlarının farklı olduğu görülmüştür. Dolayısıyla nonobstrüktif azoospermik hastaların testiküler sperm ekstraksiyonu işlemi öncesinde testis histoloji paterninin bilinmesinin, sperm elde edilmesinde prognostik rolü olduğunu düşünülmektedir.
