Effect of estrogen and vitamin D treatment on bone mineral density in patients with gonadal dysgenesis

Abstract

Objective: To compare the effects of estrogen versus estrogen and Vitamin D treatment on bone mineral density (BMD) in patients with gonadal dysgenesis. Institution: Süleyman Demirel University, Department of Obstetrics and Gynecology Materials and Methods: A total of 12 patients (4 with Turner’s syndrome) received daily 0.625 mg/day conjugated equine estrogens and 10 mg/day medroxy-progesterone acetate for 12 days/month during the study period. Twenty-five mg/day 12 months oral calcitriol was added to treatment after month 6. Proximal femur and lumbar vertebral bone mineral densities were evaluated and compared with each other at 0,6 and 12 months by using Mann-Whitney U test and Wilcoxon signed-rank test. Results: Increase in femur neck BMD was 0.046 ± 0.044 in month 0-6, and 0.035 ± 0.030 in month 6-12; increase in L2-L4 BMD was 0.053 ± 0.050 in month 0-6, and 0.065 ± 0.04 in month 6-12 with no significant differences (p=0.6 and p=0.7, respectively). Femur trochanter and Ward’s BMD were increased during month 0-6 (femur trochanter 0.073 ± 0.046; Ward’s 0.076 ± 0.030) compared to month 6-12 (femur trochanter 0.025 ± 0.020; Ward’s 0.025 ± 0.005) with statistical significance (P=0.05, P=0.05, respectively). Conclusion:Patients with gonadal dysgenesis may not benefit (in terms of BMD) from addition of Vitamin D to treatment. Investigations including more patients and various treatment modalities are needed.

Amaç: Gonadal disgenezili hastalarda östrojen tedavisi ve bu tedaviye D vitamini eklemenin kemik mineral dansitometrisi (KMD) üzerine etkisinin araştırılması. Materyal ve Metod: Gonadal disgenezisli 12 hasta (4’ü Turner sendromlu) çalışmaya alındı. Hastalara 1 yıl süreyle her gün 0.625 mg/gün konjuge östrojen ve ayda 12 gün 10 mg/gün medroksiprogesteron asetat verildi. Tedaviye altıncı aydan itibaren 0.25 mg/gün oral kalsitriol eklendi. Çalışma grubundaki hastaların tedavi öncesi, 6. ay ve tedavi sonrası proksimal femur ve lumbal vertebra KMD’leri ölçüldü. KMD değişiklikleri 6. ay sonu ve 1. yıl sonunda Mann-Whitney U test ve Wilcoxon signed-rank testi kullanılarak karşılaştırıldı. Bulgular: Çalışma grubunda ilk 6 ayda femur boynu KMD artışı 0.046 ± 0.044, ikinci 6 ayda 0.035 ± 0.030, L2-L4 KMD artışı ilk 6 ayda 0.053 ± 0.050 ikinci 6 ayda 0.065 ± 0.04 idi ve dönemler arasında istatistiki fark yoktu (sırasıyla P= 0.6, P=0.7). İki dönem arasında KMD artışları açısından femur trokanteri ve Ward’s alanındaki artış ilk 6 ayda ikinci 6 aya göre anlamlı olarak yüksekti (sırasıyla 0.073 ± 0.046, 0.025 ± 0.02, P= 0.05; 0.076 ± 0.030, 0.025 ± 0.005, P=0.05). Sonuçlar:Gonadal disgenezili hastalarda östrojen tedavisi KMD’yi artırıken, tedaviye D vitamini eklenmesi KMD’de anlamlı ek değişiklik oluşturmamaktadır. Gonadal disgenezide geniş serili ve yalnız östrojen tedavisi veya kombinasyon tedavisinin etkinliğinin karşılaştırıldığı yeni çalışmalara ihtiyaç vardır.
