The evaluation of sexual function disorders in infertile males

Abstract

Objective: To evaluate the sexual function and ejaculation disorders in infertile males Materials and Methods: 538 males who were admitted to our department between the period September 1999 and November 2003, with the initial diagnosis of male infertility were included in this prospective study. All patients were asked for complete history and underwent detailed physical examination. All patients had. semen analysis during admission. All patients were also checked for serum FSH, LH, Prolactin and free testosterone levels, and these patients were evaluated for ejaculation disorders, erectile dysfunction and loss of libido. Results: The mean age of the study group was 32±5 ( range 21-48). 116 out of 538 patients (21,5%), reported the history of sexual function disorders after clinical examination. Of these 116 patients, 65 (56%) had sperm analysis demonstrating subfertility (sperm count less than 20 million/ml); 51 (44%) had the diagnosis of azospermia in sperm analysis. Related with the sexual function disorders; 26 of the 116 patients (22%) revealed additional of erectile dysfunction; 67 patients (57%) with the diagnosis of premature ejaculation; 4 patients (3%) with anejaculation; 5 patients (4%) with retrograde ejaculation; 4 patients (3%) with retarded ejaculation and 10 patients (8%) with loss of libido, respectively.Conclusion: infertile males could have concomitant sexual function disorders like loss of libido; premature ejaculation and erectile dysfunction associated either with the organic or the psychogenic etiology, in selected group of patients with the treatment of these disorders, fertility could be achieved and can lead to the resolution of the infertility problem. Thus, in infertile couples, the sexual dysfunction must be questioned with detail and must be treated certainly.

Amaç: İnfertil hastalarda eşlik eden seksüel fonksiyon ve ejakülasyon bozukluklarının saptanması. Materyal ve Metod: Eylül 1999-Kasım 2003 tarihleri arasında bölümümüze infertilite sebebiyle başvuran ve erkek infertilitesi tanısı konulan 538 hasta bu prospektif çalışmaya alınmıştır. Tüm hastalardan ayrıntılı hikaye alınmış ve fizik muayeneleri yapılmıştır. Tüm hastaların ilk başvuru anında rutin olarak semen analizleri yapılmıştır. Ayrıca tüm hastaların serum FSH, LH, Prolaktin ve serbest testosteron düzeyleri kontrol edilmiştir. Bu hastalar ejakülasyon bozuklukları, libido azalması ve erektil disfonksiyon yönünden sorgulanmıştır.Bulgular: Çalışma grubunun yaş ortalaması 32 ± 5 (21-48). 538 hastanın 116‘sında (%21.5) cinsel sorgulama sonucunda seksüel fonksiyon bozukluğunun eşlik ettiği izlenmiştir. 116 hastanın 65’ inde (%56) spermogram sonucu subfertilite, 51(%44) hastada ise azoospermi tanısı konulmuştur. Seksüel fonksiyon bozukluğu olan 116 hastanın 26’sında (%22) erektil disfonksiyon, 67 hastada (%57) premature ejakülasyon, 4 hastada (%3) anejakülasyon, 5 hastada (%4) retrograd ejakülasyon, 4 hastada (%3) gecikmiş ejakülasyon ve 10 hastada da (%8) libido kaybının infertiliteye eşlik ettiği görülmüştür.Sonuçlar:İnfertil hastalarda psikojenik ve organik bozukluklar sebebiyle libido azalması, premature ejakülasyon ve erektil disfonksiyona bağlı infertilite izlenebilir. Belli hasta gruplarında seksüel fonksiyon bozukluklarının tedavi edilmesiyle fertilite sağlanabilmekte ve infertilite sorunu ortadan kalkabilmektedir. Bu nedenle infertil hastalarda seksüel disfonksiyon mutlaka sorgulanmalı ve tedavisi yapılmalıdır.
