Post-operative hysterosalpingography findings of patients with ectopic pregnancy treated by either radical or conservative surgery and their relation to patients' fertility

Abstract
Objective: To evaluate the patients with ectopic pregnancy following radical ( salpingectomy ) and conservative ( tube sparing ) surgery, by using hysterosalpingography (HSG) and to compare the findings of HSG to the future fertility of the patients. Institution: S.S.K. Ankara Maternity and Women's Health Teaching Hospital Materials and Methods: Twenty-two patients in radical surgery group and 13 patients in conservative surgery group were examined by HSG. Findings of HSG ( tubal patency, pelvic adhesion, and intrauterine abnormalities) were compared between the two groups. Pregnancies which occur during the mean follow-up period of 10.5 months (range 6-16 months) were also compared between the two groups. Results: Ten percent of the patients treated conservatively had tubal obstruction on HSG, whereas 27% of the patients who underwent radical surgery had tubal obstruction. Pelvic adhesion was present in 60% of the patients in radical surgery group, whereas 40% of the patients in conservative surgery group had pelvic adhesion. However, the difference between the two groups was not statistically significant (p=0.67). Of the patients who desired pregnancy, 4 of 13 patients (30%) in the conservative surgery group and 3 of 22 patients (13%) in the radical surgery group became pregnant. All the pregnancies were intrauterine. Although the pregnancy rate was higher in the conservative surgery group, the difference in pregnancy rates between the two groups was not statistically significant (p=0.38). Conclusion: The results of this study suggest that the conservative surgical approach in the treatment of ectopic pregnancy causes less pelvic adhesion formation and better reproductive results.

Amaç: Radikal (salpinjektomi) ve konservatif (tuba koruyucu) cerrahi yaklaşım uygulanan ektopik gebelik olgularının post-operatif dönemde histerosalpingografi (HSG) ile değerlendirilmesi ve bulguların gelecekteki fertilite ile karşılaştırılması. Çalışmanın Yapıldığı Yer: S.S.K. Ankara Doğumevi ve Kadın Hastalıkları Eğitim Hastanesi Materyal ve Metod: Yirmi ikisi radikal cerrahi, 13’ü konservatif cerrahi yaklaşım uygulanan toplam 35 ektopik gebelik olgusuna HSG çekildi. Tubal açıklık, pelvik adhezyon ve intrakaviter patolojiler açısından HSG bulguları iki grup arasında karşılaştırıldı. Ayrıca ortalama 10.5 aylık (6-16 ay) takip süresince meydana gelen gebelikler iki grup arasında karşılaştırıldı.Bulgular: Konservatif cerrahı yaklaşım grubunda, HSG ile hastaların %10’unda obstrüksiyon saptandı. Buna karşılık, radikal cerrahi grubunda %27 hastada obstrüksiyon bulgusu mevcuttu. Pelvik adhezyon açısından iki grup karşılaştırıldığında, radikal cerrahi yaklaşım grubunda %60, konservatif cerrahi yaklaşım uygulanan grupta %44 oranında pelvik adhezyon mevcuttu. Ancak aradaki fark istatistiksel olarak anlamlı bulunmadı (p=0.67). Gebelik isteği olan hastalardan konservatif cerrahi uygulanan grupta 13 hastanın 4’ü (%30) ve radikal cerrahi uygulanan grupta 22 hastanın 3’ü (%13) gebe kaldı. Gebeliklerin tamamı intrauterin gebelikti. Konservatif cerrahi grubunda daha yüksek oranda gebelik görülmesine rağmen, bu fark istatistiksel olarak anlamlı bulunmadı (p=0.38). Sonuçlar:Bu çalışmadan elde ettiğimiz bulgular, ektopik gebeliğin tedavisinde konservatif cerrahi yaklaşımın post-operatif dönemde daha az pelvik adhezyon oluşumuna ve daha iyi reprodüktif sonuca yolaçtığını düşündürmektedir.
